# Sompraz HP (Pantoprazole Sodium)
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach.
## Primary Indications
* Gastroesophageal reflux disease (GERD)
* Erosive esophagitis
* Zollinger-Ellison syndrome
* Part of combination therapy for *Helicobacter pylori* eradication
## Adult Dosing
* **GERD/Erosive Esophagitis:** 40 mg orally or intravenously once daily for up to 8 weeks. Maintenance therapy: 20-40 mg once daily.
* **H. pylori Eradication:** 40 mg orally twice daily for 7-14 days in combination with antibiotics (e.g., amoxicillin and clarithromycin or metronidazole and clarithromycin).
* **Zollinger-Ellison Syndrome:** Starting dose 40 mg orally or intravenously once daily, may be increased to a maximum of 240 mg daily in divided doses.
## Pediatric Dosing
Dosing varies significantly by indication and age. Refer to specific pediatric guidelines or product labeling for precise recommendations.
## Dose Adjustments
* **Hepatic Impairment:** Maximum dose of 40 mg orally or intravenously once daily.
* **Renal Impairment:** No dose adjustment typically needed.
## Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
## Adverse Effects
Common: Headache, diarrhea, nausea, abdominal pain, dizziness, flatulence.
Serious: *Clostridium difficile*-associated diarrhea, bone fractures (hip, wrist, spine) with long-term use, hypomagnesemia, vitamin B12 deficiency, systemic lupus erythematosus.
## Key Drug Interactions
* **Clopidogrel:** Pantoprazole may decrease the efficacy of clopidogrel. Avoid concomitant use if possible. If necessary, consider alternative PPIs or separate administration.
* **Methotrexate:** PPIs may increase methotrexate levels. Consider holding PPIs during methotrexate treatment.
* **Drugs dependent on gastric pH for absorption:** e.g., ketoconazole, itraconazole, certain antiretrovirals (atazanavir, nelfinavir). Pantoprazole may decrease absorption.
* **CYP2C19 Substrates:** Pantoprazole is a moderate inhibitor of CYP2C19. Use caution with other CYP2C19 substrates.
## Monitoring
* Magnesium levels, especially in patients on long-term therapy or concomitant diuretic use.
* Bone mineral density screening may be considered for patients at risk for osteoporosis.
* Monitor for signs/symptoms of *C. difficile* infection.
* For *H. pylori* eradication, confirm eradication after treatment.
## Clinical Pearls
* Administer oral pantoprazole at least 30 minutes before a meal.
* IV administration should be over 2 minutes for pantoprazole sodium for injection.
* Long-term PPI use has been associated with certain adverse events; weigh risks and benefits.
