# Sompraz HP
## Overview
Sompraz HP is a combination product containing lansoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
* Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
* One kit, taken twice daily for 14 days. Each kit contains:
* Lansoprazole 30 mg capsules
* Amoxicillin 500 mg capsules
* Clarithromycin 250 mg tablets
## Pediatric Dosing
* The use of Sompraz HP in pediatric patients is not established. Dosing for individual components in pediatric patients should follow specific guidelines for each drug.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment is recommended for lansoprazole, amoxicillin, or clarithromycin in mild to moderate renal impairment. Severe renal impairment may require dose adjustments for amoxicillin and clarithromycin. Consult specific drug monographs for guidance.
* **Hepatic Impairment:** No specific dose adjustment is recommended for lansoprazole. Clarithromycin requires dose adjustment in moderate to severe hepatic impairment. Consult specific drug monographs for guidance.
## Contraindications
* Hypersensitivity to lansoprazole, amoxicillin, clarithromycin, or any component of the formulation.
* History of severe hypersensitivity reactions to penicillins, cephalosporins, or macrolides.
* Concomitant use with cisapride, dihydroergotamine, ergotamine, pimozide, or simvastatin due to potential for serious drug interactions.
* History of cholestatic jaundice or hepatic dysfunction associated with prior clarithromycin use.
## Adverse Effects
* **Common:** Diarrhea, taste perversion, headache, nausea, abdominal pain, rash.
* **Serious:** *Clostridioides difficile*-associated diarrhea, severe allergic reactions (anaphylaxis, Stevens-Johnson syndrome), *C. difficile* colitis, liver dysfunction, QT prolongation (clarithromycin), *C. albicans* overgrowth.
## Key Drug Interactions
* **Lansoprazole:** May decrease the absorption of drugs requiring an acidic environment (e.g., ketoconazole, itraconazole, iron salts).
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. Probenecid decreases renal tubular secretion of amoxicillin.
* **Clarithromycin:** Potent inhibitor of CYP3A4. Can increase serum concentrations of drugs metabolized by CYP3A4 (e.g., statins, warfarin, theophylline, carbamazepine, calcium channel blockers). Can prolong QT interval, increasing risk of arrhythmias with other QT-prolonging agents.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for diarrhea and signs of *C. difficile* infection.
* Assess for signs and symptoms of hepatic dysfunction.
* Confirm *H. pylori* eradication post-treatment as per local guidelines (e.g., urea breath test, stool antigen test).
## Clinical Pearls
* Administer the components of Sompraz HP as a complete regimen.
* Lansoprazole should be taken before a meal.
* Ensure patient compliance with the full 14-day course to maximize eradication rates.
* Consider alternative regimens if penicillin allergy is present.
