## Sompraz
### Overview
Sompraz is a brand name for pantoprazole, a proton pump inhibitor (PPI).
### Primary Indications
* Healing of erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Maintenance of healing of erosive esophagitis.
* Treatment of pathological hypersecretory conditions, including Zollinger-Ellison syndrome.
### Adult Dosing
* **Healing of Erosive Esophagitis:** 40 mg once daily for up to 8 weeks.
* **Maintenance of Healing of Erosive Esophagitis:** 40 mg once daily.
* **Pathological Hypersecretory Conditions (e.g., Zollinger-Ellison Syndrome):** Starting dose is typically 40 mg twice daily. Doses may be increased as clinically indicated. Doses greater than 40 mg twice daily should be administered with caution. The maximum recommended dose is 120 mg twice daily or 240 mg once daily.
### Pediatric Dosing
* **Healing of Erosive Esophagitis (12 to 17 years):** 40 mg once daily for up to 8 weeks.
* **Erosive Esophagitis (less than 12 years):** Dosing is not established.
### Dose Adjustments
* **Hepatic Impairment:** For patients with severe hepatic impairment, a dose of 20 mg once daily is recommended.
### Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
### Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, dizziness, and flatulence. Long-term use of PPIs may be associated with an increased risk of fractures (hip, wrist, spine), Clostridium difficile infection, vitamin B12 deficiency, and hypomagnesemia.
### Key Drug Interactions
* **Drugs Dependent on Gastric pH for Absorption:** May decrease absorption of drugs like ketoconazole, itraconazole, posaconazole, erlotinib, and nelfinavir.
* **Methotrexate:** PPIs may increase methotrexate levels, potentially leading to toxicity.
* **CYP2C19 Substrates:** Pantoprazole is a moderate inhibitor of CYP2C19. Caution is advised when co-administered with drugs metabolized by CYP2C19, such as clopidogrel.
### Monitoring
* Monitor for signs and symptoms of Clostridium difficile-associated diarrhea.
* Consider monitoring serum magnesium levels, especially with prolonged therapy (typically >1 year) or concurrent use of other medications that lower magnesium (e.g., diuretics, digoxin).
* Monitor for signs of bone fracture.
### Clinical Pearls
* Administer pantoprazole delayed-release tablets at least 30 minutes before a meal.
* For Zollinger-Ellison syndrome, individualize the dose based on response and titration.
