# Sompraz HP
## Overview
Sompraz HP is a combination product containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* (*H. pylori*).
## Primary Indications
Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
* **Sompraz HP:** Take one blister pack (containing pantoprazole 40 mg, amoxicillin 500 mg, and clarithromycin 250 mg) twice daily for 7 days. The amoxicillin and clarithromycin capsules should be taken together twice daily, and the pantoprazole delayed-release tablet should be taken once daily.
* **Alternative Dosing (if not using combination pack):**
* Pantoprazole 40 mg orally twice daily.
* Amoxicillin 1000 mg orally twice daily.
* Clarithromycin 500 mg orally twice daily.
* *Note:* Dosing for *H. pylori* eradication can vary; follow local guidelines or prescribing information.
## Pediatric Dosing
Dosing for pediatric patients is not established for this combination product. Treatment regimens should be individualized based on available pediatric data for the individual components.
## Dose Adjustments
* **Renal Impairment:** Amoxicillin dose may need adjustment in severe renal impairment (CrCl < 30 mL/min). Clarithromycin dose should be reduced by half in severe renal impairment. Pantoprazole dose adjustment is generally not required for renal impairment.
* **Hepatic Impairment:** Caution with clarithromycin in moderate to severe hepatic impairment. Pantoprazole dose reduction may be considered in severe hepatic impairment.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or any component of the formulation.
* History of cholestatic jaundice/hepatic dysfunction associated with prior macrolide exposure.
* Concurrent use of pimozide, cisapride, and ergotamine alkaloids with clarithromycin.
* Severe renal impairment with clarithromycin.
* Viral infections.
## Adverse Effects
Common adverse effects include diarrhea, nausea, abdominal pain, headache, and taste perversion. Clarithromycin can cause QT prolongation. Amoxicillin can cause rash and hypersensitivity reactions.
## Key Drug Interactions
* **Clarithromycin:** Can inhibit CYP3A4, increasing levels of statins, certain antiarrhythmics (e.g., quinidine), benzodiazepines, colchicine, and others. Potentially enhances effects of warfarin. Increased risk of myopathy with HMG-CoA reductase inhibitors.
* **Pantoprazole:** May decrease absorption of drugs requiring acidic gastric pH (e.g., ketoconazole, itraconazole, certain HIV protease inhibitors). May increase levels of methotrexate.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
## Monitoring
* Monitor for signs of hypersensitivity reactions.
* Assess for diarrhea and *Clostridioides difficile*-associated diarrhea.
* Evaluate liver function tests, especially with clarithromycin.
* Monitor renal function if indicated.
## Clinical Pearls
* This combination therapy is intended for a short duration (typically 7 days).
* Ensure the patient understands the full dosing regimen for all three components.
* Educate patients on potential side effects and when to seek medical attention.
* *H. pylori* eradication should be confirmed with a urea breath test, stool antigen test, or biopsy after treatment.
