# Sompraz HP (Pantoprazole)
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that reduces gastric acid production.
## Primary Indications
* Erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Healing of erosive esophagitis and reduction of associated heartburn.
* Pathological hypersecretory conditions, including Zollinger-Ellison syndrome.
## Adult Dosing
* **Erosive Esophagitis (Healing):** 40 mg orally once daily for up to 8 weeks. A subsequent 8-week course may be considered for patients who have not healed.
* **Maintenance of Healing of Erosive Esophagitis and Reduction of Associated Heartburn:** 40 mg orally once daily.
* **GERD (Symptomatic Relief):** 20 mg orally once daily for up to 4 weeks. If symptoms do not resolve, consider other causes.
* **Pathological Hypersecretory Conditions (e.g., Zollinger-Ellison Syndrome):** Starting dose is typically 40 mg orally twice daily. Doses may be adjusted based on patient response and need. Doses up to 240 mg daily have been administered intravenously.
## Pediatric Dosing
Dosing in pediatric patients is not established for all indications and may vary based on age and weight. Consult specific pediatric guidelines or prescribing information.
* **GERD (12 years and older):** 40 mg orally once daily for up to 8 weeks.
## Dose Adjustments
No specific dose adjustment is recommended for patients with hepatic or renal impairment.
## Contraindications
Known hypersensitivity to pantoprazole or any component of the formulation.
## Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, vomiting, dizziness, and rash.
## Key Drug Interactions
* **Drugs Dependent on Gastric pH for Absorption:** Pantoprazole may decrease the absorption of drugs such as ketoconazole, itraconazole, and certain HIV protease inhibitors (e.g., atazanavir, nelfinavir).
* **Methotrexate:** PPIs may increase methotrexate levels, potentially leading to toxicity. Consider dose reduction or temporary discontinuation.
* **CYP2C19 Substrates:** Pantoprazole is a moderate inhibitor of CYP2C19. Use with caution with other drugs metabolized by this enzyme.
## Monitoring
* Monitor for signs and symptoms of fracture (e.g., hip, wrist, spine), Clostridium difficile-associated diarrhea, and low magnesium levels, especially with prolonged use (>1 year).
* Assess for therapeutic effectiveness.
## Clinical Pearls
* Administer pantoprazole delayed-release tablets 30 minutes before a meal.
* Do not crush or chew delayed-release tablets.
* Prolonged use of PPIs is associated with an increased risk of bone fractures, Clostridium difficile infection, and vitamin B12 deficiency.
