# Sompraz HP
## Overview
Sompraz HP is a combination product containing **omeprazole**, **amoxicillin**, and **clarithromycin** used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
* Eradication of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
* **Sompraz HP:** One capsule pack taken twice daily for 10 to 14 days. Each pack typically contains:
* Omeprazole 20 mg delayed-release capsules
* Amoxicillin 500 mg capsules
* Clarithromycin 250 mg tablets
* **Administration:** Take all medications as directed. Omeprazole should be taken before a meal. Amoxicillin and clarithromycin are typically taken with meals to minimize gastrointestinal upset.
## Pediatric Dosing
* Dosing for pediatric patients is not established for this combination product. Dosing of individual components may be considered based on weight and clinical judgment, but specific regimens are not readily available for this fixed combination.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment for omeprazole, amoxicillin, or clarithromycin is typically recommended in mild to moderate renal impairment. In severe renal impairment, caution is advised, and dose adjustments for amoxicillin and clarithromycin may be necessary based on individual component guidelines.
* **Hepatic Impairment:** For omeprazole, a reduction in dose may be considered in patients with severe hepatic impairment. For clarithromycin, dose reduction is recommended in moderate to severe hepatic impairment.
## Contraindications
* Hypersensitivity to omeprazole, amoxicillin, clarithromycin, or other macrolides or penicillins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with ergotamine or dihydroergotamine.
* Severe renal impairment (CrCl < 30 mL/min) for clarithromycin may warrant dose adjustment.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, abdominal pain, taste perversion, headache.
* **Serious:** *Clostridioides difficile*-associated diarrhea, severe allergic reactions (anaphylaxis), *Stevens-Johnson syndrome*, *toxic epidermal necrolysis*, hepatotoxicity, QT prolongation (clarithromycin), *hemolytic anemia* (amoxicillin).
## Key Drug Interactions
* **Omeprazole:** Affects absorption of drugs requiring acidic environment (e.g., ketoconazole, itraconazole, atazanavir). May increase levels of drugs metabolized by CYP2C19 and CYP3A4 (e.g., clopidogrel, diazepam, warfarin).
* **Clarithromycin:** Potent CYP3A4 inhibitor. Significantly increases levels of drugs metabolized by CYP3A4 (e.g., statins, calcium channel blockers, certain antiretrovirals, warfarin). Avoid concurrent use with colchicine, pimozide, cisapride, and simvastatin/lovastatin. May increase levels of theophylline and carbamazepine.
* **Amoxicillin:** May decrease efficacy of oral contraceptives. Probenecid may increase and prolong blood levels of amoxicillin.
## Monitoring
* Assess for signs and symptoms of *H. pylori* eradication (e.g., symptom improvement).
* Monitor for adverse effects, particularly diarrhea, rash, and signs of hypersensitivity.
* Consider stool testing for *C. difficile* if diarrhea occurs.
* Liver function tests may be considered, especially with prolonged therapy or in patients with hepatic impairment.
## Clinical Pearls
* This combination therapy provides a convenient "all-in-one" package for *H. pylori* eradication.
* Patient adherence is crucial for successful eradication. Counsel patients on the importance of completing the full course of therapy.
* Ensure patients understand the dosing schedule for each component.
