# Sompraz HP
## Overview
Sompraz HP is a combination product containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
* Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
* **Sompraz HP:** Each unit contains pantoprazole 40 mg, amoxicillin 500 mg, and clarithromycin 500 mg.
* **Dosing Regimen:** Take one unit (containing all three medications) twice daily for 7 days.
## Pediatric Dosing
* Data on pediatric use of Sompraz HP is limited. Dosing should be based on individual patient factors and expert recommendation.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment is recommended for pantoprazole or clarithromycin in mild to moderate renal impairment. In severe renal impairment, consider reduced doses of amoxicillin and clarithromycin.
* **Hepatic Impairment:** No specific dose adjustment is recommended for pantoprazole in mild to moderate hepatic impairment. Use with caution in severe hepatic impairment. Consider dose reduction for clarithromycin in significant hepatic impairment.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or any component of the formulation.
* History of hypersensitivity to penicillins, cephalosporins, or macrolides.
* History of cholestatic jaundice or hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with rifampin, astemizole, cisapride, ergotamine, methylergonovine, pimozide, or simvastatin/lovastatin.
## Adverse Effects
* **Common:** Diarrhea, nausea, abdominal pain, taste disturbance, headache.
* **Serious:** *Clostridium difficile*-associated diarrhea, hypersensitivity reactions (including anaphylaxis), severe skin reactions (e.g., Stevens-Johnson syndrome), liver injury, QT prolongation, dysglycemia (especially in patients with diabetes).
## Key Drug Interactions
* **Pantoprazole:** Can decrease the absorption of drugs requiring acidic environments (e.g., ketoconazole, itraconazole, certain HIV protease inhibitors). May interact with warfarin.
* **Clarithromycin:** Potent inhibitor of CYP3A4. Numerous interactions with drugs metabolized by CYP3A4, including statins (risk of myopathy), colchicine (risk of toxicity), calcium channel blockers, benzodiazepines, and anticoagulants (e.g., warfarin, apixaban). May interact with theophylline and digoxin.
* **Amoxicillin:** May interfere with urine glucose and urobilinogen tests.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for *Clostridium difficile*-associated diarrhea.
* Monitor liver function tests, especially with prolonged use or in patients with pre-existing hepatic disease.
* Monitor for QT interval prolongation, particularly in patients with risk factors.
* Monitor blood glucose in diabetic patients.
## Clinical Pearls
* Administer Sompraz HP as prescribed for the full 7-day duration, even if symptoms improve.
* Ensure patients are instructed on proper administration, including taking the unit as a whole.
* Advise patients to report any severe diarrhea, rash, or signs of allergic reaction immediately.
* Consider alternative *H. pylori* eradication regimens in areas with high clarithromycin resistance rates.
