## Sompraz (Pantoprazole)
### Overview
Pantoprazole is a proton pump inhibitor (PPI) that decreases gastric acid production.
### Primary Indications
* Healing of erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Maintenance of healing of erosive esophagitis.
* Treatment of pathological hypersecretory conditions, including Zollinger-Ellison syndrome.
### Adult Dosing
* **Erosive esophagitis:** 40 mg orally or intravenously once daily for up to 8 weeks. A subsequent 8-week course may be considered for patients who have not healed.
* **Maintenance of healing erosive esophagitis:** 40 mg orally once daily.
* **Pathological hypersecretory conditions:** Starting dose is typically 40 mg orally or intravenously twice daily. Doses can be increased to 80 mg intravenously or orally every 8 hours. Doses up to 240 mg daily have been administered.
### Pediatric Dosing
* **Erosive esophagitis (age 5 to 16 years):** 20 mg or 40 mg orally once daily for up to 8 weeks.
* **Erosive esophagitis (age less than 5 years):** Dosing varies and is often weight-based. For example, 1.0 to 2.0 mg/kg/day orally divided twice daily. Exact pediatric dosing should be guided by specific guidelines and patient weight.
### Dose Adjustments
* **Hepatic Impairment:** Maximum daily dose of 40 mg orally.
### Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
### Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, vomiting, dizziness, and rash. Long-term use of PPIs may be associated with an increased risk of bone fractures, Clostridium difficile infection, and vitamin B12 deficiency.
### Key Drug Interactions
* **Drugs requiring gastric acid for absorption:** Pantoprazole can decrease the absorption of drugs like ketoconazole, itraconazole, and certain HIV protease inhibitors (e.g., atazanavir).
* **Methotrexate:** PPIs may increase methotrexate levels.
### Monitoring
* Monitor for effectiveness in symptom relief.
* Monitor for adverse effects, especially with long-term use.
* Consider vitamin B12 levels with prolonged therapy.
### Clinical Pearls
* Pantoprazole is generally more stable in acidic conditions than some other PPIs, but it should still be taken before a meal.
* For IV administration, reconstitute and dilute as per manufacturer instructions.
* The maximum dose for erosive esophagitis is 40 mg once daily. Higher doses are used for hypersecretory conditions.
