# Sompraz HP
## Overview
Sompraz HP is a fixed-dose combination product containing lansoprazole, amoxicillin, and clarithromycin. It is a proton pump inhibitor (PPI) and two antibiotics used for the eradication of *Helicobacter pylori* (*H. pylori*) infection.
## Primary Indications
* *Helicobacter pylori* eradication in patients with peptic ulcer disease or a history of peptic ulcer disease.
## Adult Dosing
The typical adult dose is one Sompraz HP capsule taken orally twice daily for 7 to 14 days. The specific duration of therapy depends on local guidelines and clinical judgment.
## Pediatric Dosing
There is no established pediatric dosing for Sompraz HP. Dosing for pediatric patients with *H. pylori* infection is individualized based on age, weight, and the specific components of the regimen.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment is recommended for lansoprazole in mild to moderate renal impairment. However, amoxicillin and clarithromycin may require dose adjustments in severe renal impairment. Consult specific product monographs for amoxicillin and clarithromycin for detailed guidance.
* **Hepatic Impairment:** Lansoprazole is extensively metabolized by the liver. Use with caution in patients with severe hepatic impairment. Clarithromycin should be used with caution in patients with moderate to severe hepatic impairment. No specific dose adjustment is recommended for amoxicillin.
## Contraindications
* Known hypersensitivity to lansoprazole, amoxicillin, clarithromycin, or any component of the formulation.
* History of allergic reactions to penicillins or cephalosporins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with cisapride, pimozide, or other drugs that are substrates of CYP3A4 and can prolong the QT interval.
* Severe renal impairment with concomitant clarithromycin (if applicable based on specific product).
## Adverse Effects
Common adverse effects include diarrhea, taste disturbance, headache, nausea, abdominal pain, rash, and candidiasis. Serious adverse effects can include severe allergic reactions, *Clostridioides difficile*-associated diarrhea, liver dysfunction, and QT prolongation (associated with clarithromycin).
## Key Drug Interactions
* **Lansoprazole:** Can affect absorption of drugs dependent on gastric pH (e.g., ketoconazole, itraconazole, iron salts). May increase theophylline levels.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. Probenecid may increase amoxicillin levels.
* **Clarithromycin:** A potent inhibitor of CYP3A4. Can increase plasma concentrations of drugs metabolized by CYP3A4 (e.g., warfarin, carbamazepine, cyclosporine, tacrolimus, simvastatin, lovastatin, rifabutin, midazolam, triazolam, cisapride, pimozide). Avoid concurrent use with colchicine due to increased risk of toxicity.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* Assess for signs of *Clostridioides difficile*-associated diarrhea.
* Monitor liver function tests, especially in patients with pre-existing hepatic disease or during prolonged therapy.
* Assess for efficacy of *H. pylori* eradication with appropriate testing after completion of therapy (e.g., urea breath test, stool antigen test).
## Clinical Pearls
* Sompraz HP should be taken as prescribed for the full duration of therapy to ensure eradication of *H. pylori*.
* Patients should be advised to report any new or worsening symptoms, including severe diarrhea, rash, or signs of liver problems.
* This combination therapy is one of several accepted regimens for *H. pylori* eradication. The choice of regimen may depend on local antibiotic resistance patterns and patient factors.
