# Sompraz HP
## Overview
Sompraz HP is a combination product containing lansoprazole, amoxicillin, and clarithromycin, indicated for the eradication of *Helicobacter pylori* infection. Lansoprazole is a proton pump inhibitor (PPI), while amoxicillin and clarithromycin are antibiotics.
## Primary Indications
Eradication of *Helicobacter pylori* infection in patients with duodenal or gastric ulcer disease.
## Adult Dosing
* **Sompraz HP:** One bottle, taken orally twice daily for 7 days. Each bottle contains lansoprazole 30 mg, amoxicillin 1 gram, and clarithromycin 500 mg.
## Pediatric Dosing
Pediatric dosing for Sompraz HP is not established. Individual components may have pediatric dosing guidelines.
## Dose Adjustments
No specific dose adjustments are routinely recommended for renal or hepatic impairment for the combination product. However, individual component guidelines should be consulted if concerns arise.
## Contraindications
* Hypersensitivity to any of the active ingredients or excipients.
* History of hypersensitivity to penicillin, cephalosporins, or macrolide antibiotics.
* Concomitant use with astemizole, cisapride, ergotamine, pimozide, or terfenadine.
## Adverse Effects
Common adverse effects include diarrhea, headache, nausea, abdominal pain, taste perversion, and rash. More serious effects can include *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions, and QT prolongation (especially with clarithromycin).
## Key Drug Interactions
* **Lansoprazole:** May affect absorption of drugs requiring acidic environments (e.g., ketoconazole, itraconazole, iron salts). May increase levels of drugs metabolized by CYP2C19 (e.g., phenytoin, warfarin).
* **Clarithromycin:** Significant interactions with CYP3A4 substrates (e.g., statins, calcium channel blockers, benzodiazepines). Can increase serum concentrations of digoxin and cyclosporine. Potential for QT prolongation with other QT-prolonging agents.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for diarrhea, especially if severe or persistent, to rule out *C. difficile*.
* Assess for signs of therapeutic response and ulcer healing.
## Clinical Pearls
* Administer the entire contents of one bottle twice daily for 7 days.
* Ensure patient adherence to the full 7-day course to maximize eradication rates and minimize resistance development.
* Consider alternative regimens if penicillin allergy is present or if local resistance patterns for clarithromycin are high.
