# Sompraz HP (Rabeprazole Sodium)
## Overview
Rabeprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion.
## Primary Indications
* Healing of erosive esophagitis
* Maintenance of healing of erosive esophagitis
* Symptomatic gastroesophageal reflux disease (GERD)
* H. pylori eradication (in combination with antibiotics)
* Prevention of NSAID-induced gastric ulcers
## Adult Dosing
* **Healing of erosive esophagitis:** 20 mg orally once daily for 4 to 8 weeks.
* **Maintenance of healing of erosive esophagitis:** 10 mg to 20 mg orally once daily. The dose should be individualized.
* **Symptomatic GERD:** 20 mg orally once daily for 4 weeks. If symptoms do not resolve, consider an additional 4 weeks.
* **H. pylori eradication:** 20 mg orally twice daily for 7 days in combination with appropriate antibiotics (e.g., amoxicillin and clarithromycin).
* **Prevention of NSAID-induced gastric ulcers:** 10 mg or 20 mg orally once daily.
## Pediatric Dosing
* **Symptomatic GERD:** Safety and efficacy not established in pediatric patients.
* **Erosive esophagitis:** Safety and efficacy not established in pediatric patients.
## Dose Adjustments
No dose adjustment is generally required for patients with renal impairment or hepatic impairment. However, caution and close monitoring may be warranted in severe hepatic impairment.
## Contraindications
* Known hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation.
## Adverse Effects
Common adverse effects include headache, diarrhea, abdominal pain, nausea, vomiting, and dizziness.
## Key Drug Interactions
* **Drugs dependent on gastric pH for absorption:** PPIs can decrease gastric pH, potentially affecting the absorption of drugs like ketoconazole, itraconazole, and certain HIV protease inhibitors.
* **Methotrexate:** PPIs may increase and prolong serum levels of methotrexate, potentially leading to toxicity.
* **CYP2C19 substrates:** Rabeprazole is a moderate inhibitor of CYP2C19. Concurrent use with clopidogrel may lead to decreased exposure to clopidogrel's active metabolite, potentially reducing its antiplatelet effect.
## Monitoring
* Monitor for symptomatic improvement.
* Monitor for signs and symptoms of H. pylori infection if used for eradication.
* Consider monitoring electrolytes (e.g., magnesium) with long-term PPI therapy.
## Clinical Pearls
* Rabeprazole may be taken with or without food.
* For H. pylori eradication, ensure completion of the full antibiotic course.
* Long-term use of PPIs has been associated with an increased risk of bone fractures, Clostridium difficile-associated diarrhea, and vitamin B12 deficiency.
