## Overview

## Adult Dosing
**Sompraz HP** contains:
* Omeprazole 20 mg
* Amoxicillin 1000 mg (given as two 500 mg capsules)

## Pediatric Dosing
Sompraz HP is **not approved** for use in pediatric patients. ## Dose Adjustments

## Contraindications

* **Clarithromycin:** Potent inhibitor of CYP3A4. May increase serum concentrations of numerous drugs including statins (e.g., simvastatin, lovastatin), warfarin, theophylline, certain benzodiazepines, calcium channel blockers, and colchicine. Contraindicated with cisapride, pimozide, astemizole, and quinidine.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. Probenecid may increase and prolong serum levels of amoxicillin.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* Monitor for diarrhea, particularly *C. difficile*-associated diarrhea.
* Assess for efficacy of *H. pylori* eradication via urea breath test, stool antigen test, or endoscopy (typically 4-6 weeks post-treatment).
* Evaluate hepatic function periodically, especially with prolonged use or in patients with pre-existing liver disease.
## Clinical Pearls
* Administer the entire contents of the Sompraz HP pack twice daily for 10 to 14 days.
* Omeprazole should be taken before a meal.
* It is crucial to complete the entire course of therapy to ensure eradication of *H. pylori*.
* Patients should be advised to report any severe diarrhea or signs of a severe allergic reaction immediately.
* Consider local *H. pylori* resistance patterns when selecting or confirming the appropriateness of this regimen.
