# Sompraz HP
## Overview
Sompraz HP is a combination product containing **esomeprazole**, **amoxicillin**, and **clarithromycin**. Esomeprazole is a proton pump inhibitor (PPI). Amoxicillin and clarithromycin are antibiotics.
## Primary Indications
Sompraz HP is indicated for the eradication of *Helicobacter pylori* (*H. pylori*) infection in patients with active peptic ulcer disease or a history of peptic ulcer disease.
## Adult Dosing
The typical adult dose is one (1) packet twice daily for 10-14 days. Each packet contains esomeprazole 20 mg, amoxicillin 1000 mg, and clarithromycin 500 mg.
## Pediatric Dosing
Pediatric dosing information for Sompraz HP is not established. Dosing for individual components should be used if prescribed separately for pediatric patients, with careful consideration of weight-based recommendations and potential risks.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment is recommended for esomeprazole or clarithromycin in mild to moderate renal impairment. Severe renal impairment may require dose adjustment for amoxicillin. Consult product-specific information for amoxicillin.

## Key Drug Interactions
* **Esomeprazole:** May affect the absorption of drugs that require acidic gastric pH (e.g., ketoconazole, itraconazole, certain HIV protease inhibitors). May increase serum concentrations of certain drugs metabolized by CYP2C19 or CYP3A4 (e.g., warfarin, diazepam, phenytoin, voriconazole).
* **Amoxicillin:** May decrease the effectiveness of oral contraceptives.
* **Clarithromycin:** Potentiates the effects of warfarin. May increase serum concentrations of drugs metabolized by CYP3A4 (e.g., carbamazepine, theophylline, cyclosporine, tacrolimus, midazolam, certain statins, colchicine). Increases risk of QT prolongation and arrhythmias when used with other QT-prolonging drugs.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* Assess for effectiveness in *H. pylori* eradication (e.g., urea breath test, stool antigen test, or biopsy after treatment completion).
* Monitor for *Clostridium difficile*-associated diarrhea.
* Monitor liver function tests, especially with prolonged use or in patients with pre-existing liver disease.
## Clinical Pearls
* Administer Sompraz HP as a single packet, twice daily, as prescribed.
* The entire contents of the packet should be mixed with a small amount of water (e.g., 1 tablespoon) and stirred well before administration. The mixture should be consumed immediately. Do not chew or crush the granules.
* Ensure patients complete the full course of therapy to optimize *H. pylori* eradication rates.
* Advise patients to report any severe or persistent diarrhea, rash, or signs of liver dysfunction.
* Consider potential for cross-reactivity with penicillin allergy.
