# Sompraz HP
## Overview
Sompraz HP is a combination product containing lansoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori*.
## Primary Indications
Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
**Sompraz HP:** Take one capsule (containing lansoprazole 30 mg, amoxicillin 500 mg, and clarithromycin 250 mg) twice daily for 14 days.
## Pediatric Dosing
Pediatric dosing for Sompraz HP is not specifically established. Dosing for individual components should be considered based on age, weight, and severity of infection, guided by specific pediatric guidelines.
## Dose Adjustments
No dose adjustments are typically required for hepatic or renal impairment for this combination product. However, individual component recommendations may vary.
## Contraindications
Hypersensitivity to lansoprazole, amoxicillin, clarithromycin, or any component of the formulation. History of cholestatic jaundice or hepatic dysfunction associated with prior clarithromycin use. Use with cisapride, pimozide, or other drugs that prolong the QT interval.
## Adverse Effects
Common adverse effects include diarrhea, nausea, abdominal pain, taste perversion, and headache. More serious potential adverse effects include *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions, hepatic dysfunction, and QT prolongation.
## Key Drug Interactions
* **Lansoprazole:** May decrease the absorption of drugs that require an acidic environment (e.g., ketoconazole, digoxin). May interact with CYP2C19 and CYP3A4 substrates.
* **Clarithromycin:** Potent inhibitor of CYP3A4, leading to increased concentrations of many drugs metabolized by this enzyme (e.g., statins, certain calcium channel blockers, anticoagulants, colchicine). May prolong the QT interval when used with other QT-prolonging agents.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
## Monitoring
Monitor for signs and symptoms of hypersensitivity reactions, *C. difficile* infection, and hepatic dysfunction. Monitor patients for improvement in ulcer symptoms.
## Clinical Pearls
* Administer as directed for the full 14-day course, even if symptoms improve sooner.
* This combination is one of several regimens used for *H. pylori* eradication. Resistance patterns may influence efficacy.
* Ensure patients are aware of potential interactions, particularly with clarithromycin.
