# Sompraz HP
## Overview
Sompraz HP is a combination product containing dexlansoprazole, lansoprazole, and amoxicillin, or dexlansoprazole, lansoprazole, and clarithromycin, intended for the eradication of *Helicobacter pylori* infection. Dexlansoprazole and lansoprazole are proton pump inhibitors (PPIs). Amoxicillin and clarithromycin are antibiotics.
## Primary Indications
* Eradication of *Helicobacter pylori* in patients with duodenal or gastric ulcer disease.
## Adult Dosing
The typical adult regimen is one Sompraz HP pack taken twice daily for 10-14 days. Each pack contains:
* Dexlansoprazole 60 mg capsule
* Lansoprazole 30 mg capsule
* Amoxicillin 1000 mg capsule OR Clarithromycin 500 mg capsule (depending on the specific formulation)

## Contraindications

* Stevens-Johnson syndrome/toxic epidermal necrolysis
## Key Drug Interactions
* **PPIs:** Can decrease absorption of drugs requiring acidic environments (e.g., ketoconazole, itraconazole, certain HIV protease inhibitors). May increase levels of digoxin.
* **Clarithromycin:** Strong CYP3A4 inhibitor. Increases levels of numerous drugs including statins, warfarin, certain antiarrhythmics, and others.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of *H. pylori* eradication (e.g., resolution of ulcer symptoms).
* Monitor for adverse effects, particularly diarrhea, rash, and signs of liver dysfunction.
* Assess for drug interactions, especially with clarithromycin.
## Clinical Pearls
* Administer Sompraz HP as a complete pack. Do not separate the capsules.
* Take twice daily for the full duration of therapy (10-14 days) to ensure eradication.
* Consider local resistance patterns when choosing empiric therapy.
* If a patient has a penicillin allergy, alternative *H. pylori* eradication regimens should be considered.
