# Sompraz HP (Pantoprazole HP)
## Overview
Pantoprazole HP is a proton pump inhibitor (PPI) used to reduce the production of stomach acid.
## Primary Indications
* Healing of erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Maintenance of healing of erosive esophagitis.
* Pathological hypersecretory conditions, including Zollinger-Ellison syndrome.
## Adult Dosing
* **Healing of Erosive Esophagitis:** 40 mg orally once daily for up to 8 weeks.
* **Maintenance of Healing:** 40 mg orally once daily.
* **Pathological Hypersecretory Conditions:** Starting dose is typically 40 mg orally twice daily. Doses can be increased as needed. Maximum recommended dose is 120 mg orally per day.
## Pediatric Dosing
Dosing for pediatric patients is not well-established for the HP formulation. Standard pantoprazole dosing for erosive esophagitis in pediatric patients 5 to 16 years of age is 20 mg orally once daily for up to 8 weeks. Dosing in younger children or for other indications requires careful consideration and may not be appropriate with the HP formulation.
## Dose Adjustments
No dose adjustment is typically needed for patients with hepatic or renal impairment.
## Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
## Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, vomiting, dizziness, and flatulence. Long-term PPI use may be associated with an increased risk of C. difficile infection, bone fractures, hypomagnesemia, and vitamin B12 deficiency.
## Key Drug Interactions
* **Clopidogrel:** Pantoprazole may decrease the antiplatelet effect of clopidogrel. Consider alternative PPIs or separate administration by at least 12 hours.
* **Methotrexate:** PPIs may increase methotrexate levels.
* **Drugs dependent on gastric pH for absorption:** e.g., ketoconazole, itraconazole, posaconazole, iron salts, vitamin B12.
## Monitoring
* Monitor for signs and symptoms of hypomagnesemia (e.g., tremor, muscle spasms, seizures, arrhythmias).
* Monitor for signs of Clostridium difficile-associated diarrhea.
* Assess for potential bone fractures, especially with long-term use.
## Clinical Pearls
* Pantoprazole HP is a delayed-release formulation. It should be swallowed whole and not crushed or chewed.
* It can be taken with or without food.
* Consider the potential risks associated with long-term PPI use.
