# Sompraz HP (pantoprazole)
## Overview
Sompraz HP is a fixed-dose combination product containing pantoprazole, a proton pump inhibitor (PPI), amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
The recommended dose is one Sompraz HP pack once daily for 7 days. Each pack contains:
* Pantoprazole 40 mg
* Amoxicillin 1 gram (1000 mg)
* Clarithromycin 500 mg
Patients should take all three medications as directed.
## Pediatric Dosing
Sompraz HP is not approved for use in pediatric patients. Dosing for *H. pylori* eradication in children typically involves individualized regimens based on amoxicillin and clarithromycin components, often requiring different formulations.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment for pantoprazole is typically needed. Amoxicillin and clarithromycin doses may require adjustment based on the severity of renal impairment, although the fixed-dose combination product may not allow for this. Consult specific amoxicillin and clarithromycin prescribing information.
* **Hepatic Impairment:** No dose adjustment for pantoprazole is typically needed at doses up to 40 mg daily. For doses higher than 40 mg, use with caution. Amoxicillin and clarithromycin do not typically require dose adjustment in mild to moderate hepatic impairment, but caution is advised.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or any component of the formulation.
* History of allergic reactions to penicillins or cephalosporins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior macrolide exposure.
* Concomitant use with rifabutin, rifampin, or St. John's wort due to potential for reduced efficacy of pantoprazole.
* Patients with known cardiac rhythm abnormalities, hypokalemia, or hypomagnesemia (due to clarithromycin's potential to prolong QT interval).
* Severe renal impairment (for clarithromycin component).
## Adverse Effects
Common adverse effects include diarrhea, nausea, abdominal pain, headache, rash, taste disturbances, and C. difficile-associated diarrhea. Clarithromycin can cause QT prolongation, potentially leading to serious arrhythmias.
## Key Drug Interactions
* **Pantoprazole:** May decrease the absorption of drugs requiring an acidic environment (e.g., ketoconazole, itraconazole, iron salts). May interact with warfarin, methotrexate, and drugs metabolized by CYP enzymes.
* **Clarithromycin:** Potent inhibitor of CYP3A4. Can increase serum concentrations of numerous drugs including statins (risk of myopathy), colchicine (risk of toxicity), certain calcium channel blockers, benzodiazepines, and anticoagulants (e.g., warfarin, apixaban, rivaroxaban). Avoid co-administration with pimozide, cisapride, and ergotamine alkaloids.
* **Amoxicillin:** May interact with warfarin, potentially increasing INR. May decrease the efficacy of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of *H. pylori* eradication (e.g., resolution of ulcer symptoms). Urea breath test or stool antigen test may be used to confirm eradication.
* Monitor for adverse effects, particularly diarrhea (assess for C. difficile), rash, and hypersensitivity reactions.
* Monitor electrolytes (potassium, magnesium) if risk factors for QT prolongation or hypokalemia are present.
* Monitor INR if co-administered with warfarin.
## Clinical Pearls
* Administer Sompraz HP pack once daily for 7 consecutive days.
* The three components (pantoprazole, amoxicillin, clarithromycin) should be taken together as a single daily dose.
* Ensure patients complete the full 7-day course of therapy, even if they feel better.
* Educate patients on potential adverse effects, especially diarrhea and the importance of reporting severe or persistent symptoms.
* Advise patients to report any new or worsening symptoms or any signs of allergic reaction.
