# Sompraz HP (Pantoprazole)
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach.
## Primary Indications
* Healing of erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Maintenance of healing of erosive esophagitis.
* Treatment of pathological hypersecretory conditions, including Zollinger-Ellison Syndrome (ZES).
## Adult Dosing
* **Healing of erosive esophagitis:** 40 mg orally or intravenously once daily for up to 8 weeks.
* **Maintenance of healing of erosive esophagitis:** 40 mg orally once daily.
* **Pathological hypersecretory conditions (e.g., ZES):**
* **Oral:** Starting dose is typically 40 mg orally twice daily. Doses may be increased as needed. Doses up to 240 mg daily have been administered.
* **Intravenous:** Starting dose is typically 80 mg intravenously once daily. Doses may be adjusted as needed. Doses up to 240 mg daily have been administered.
## Pediatric Dosing
* **Erosive esophagitis (age 5 years and older):** 20 mg to 40 mg orally once daily for up to 8 weeks.
* **GERD symptoms (age 5 years and older):** 20 mg orally once daily for up to 4 weeks.
## Dose Adjustments
* **Hepatic impairment:** Maximum daily dose is 40 mg orally.
## Contraindications
* Known hypersensitivity to pantoprazole or any of its components.
## Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, vomiting, dizziness, and arthralgia.
## Key Drug Interactions
* **Drugs dependent on gastric pH for absorption:** PPIs can decrease the absorption of drugs that require an acidic gastric environment (e.g., ketoconazole, itraconazole, posaconazole, ampicillin esters, iron salts, vitamin B12).
* **Methotrexate:** PPIs may increase serum levels of methotrexate.
* **CYP2C19 substrates:** Pantoprazole is a moderate inhibitor of CYP2C19, which can affect the metabolism of certain drugs (e.g., clopidogrel, though clinical significance is debated).
## Monitoring
* Monitor for signs and symptoms of infection (e.g., Clostridium difficile-associated diarrhea).
* Monitor for bone fracture risk (long-term use).
* Monitor for hypomagnesemia (long-term use).
* Monitor for vitamin B12 deficiency (long-term use).
## Clinical Pearls
* Administer oral pantoprazole 30 minutes before a meal.
* Intravenous administration is typically reserved for patients unable to take oral medication.
* Long-term PPI use may be associated with an increased risk of fractures, C. difficile infection, and hypomagnesemia.
