**Overview**
Sompraz hp is a combination medication containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection.
**Primary Indications**
* Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
**Adult Dosing**
The typical regimen is one dose packet (containing pantoprazole 40 mg, amoxicillin 1000 mg, and clarithromycin 500 mg) taken twice daily for 7 to 14 days.
* Pantoprazole: 40 mg twice daily
* Amoxicillin: 1000 mg twice daily
* Clarithromycin: 500 mg twice daily
**Pediatric Dosing**
Dosing for pediatric patients is not established for this specific combination product. Standard pediatric regimens for *H. pylori* eradication may involve different formulations and doses of the individual components, typically guided by specialist recommendations and patient weight.
**Dose Adjustments**
* **Renal Impairment:** No specific dose adjustment is routinely recommended for pantoprazole or amoxicillin in mild to moderate renal impairment. However, caution and potential dose reduction may be considered in severe renal impairment. Clarithromycin requires dose adjustment in significant renal impairment (CrCl < 30 mL/min).

**Contraindications**

* **Clarithromycin:** Strong CYP3A4 inhibitor; can increase serum concentrations of numerous drugs metabolized by CYP3A4 (e.g., statins, colchicine, certain calcium channel blockers, benzodiazepines, digoxin). Avoid co-administration with cisapride, pimozide, and ergotamine derivatives.
**Monitoring**
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for gastrointestinal adverse effects, including signs of C. difficile infection.
* Assess for therapeutic efficacy (e.g., resolution of symptoms, confirmation of *H. pylori* eradication via testing after treatment).
* Monitor liver function tests, especially with prolonged use or in patients with hepatic impairment.
* Monitor electrolytes, particularly in patients at risk for imbalance.

* Ensure patients complete the full course of therapy, even if symptoms improve.
* Advise patients on potential gastrointestinal side effects and to report severe or persistent symptoms.
