# Sompraz HP
## Overview
Sompraz HP is a combination product containing **esomeprazole** (a proton pump inhibitor or PPI) and **amoxicillin** and **clarithromycin** (antibiotics). It is designed for the eradication of *Helicobacter pylori* (*H. pylori*).
## Primary Indications
Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
The typical adult regimen is one dose pack taken twice daily for 10 days. Each dose pack usually contains:
* Esomeprazole 20 mg
* Amoxicillin 1000 mg
* Clarithromycin 500 mg
**Note:** Dosing may vary based on local resistance patterns and established treatment guidelines.
## Pediatric Dosing

* **Clarithromycin:** Potent inhibitor of CYP3A4. Can increase serum concentrations of numerous drugs including statins (simvastatin, lovastatin), colchicine, warfarin, benzodiazepines, calcium channel blockers, and others, potentially leading to toxicity. Risk of QT prolongation with antiarrhythmics, antipsychotics, and other drugs.
* **Amoxicillin:** Generally fewer significant interactions. May decrease effectiveness of oral contraceptives.
## Monitoring
* Assess for signs and symptoms of *H. pylori* eradication (e.g., resolution of ulcer symptoms).
* Monitor for signs of hypersensitivity reactions.
* Monitor for diarrhea, which could indicate *Clostridioides difficile* infection.
* Evaluate for adverse effects associated with individual components.
## Clinical Pearls
* This combination therapy is a "triple therapy" regimen for *H. pylori* eradication.
* Patients should complete the full 10-day course as prescribed, even if symptoms improve earlier.
* Take esomeprazole before a meal. Amoxicillin and clarithromycin can be taken with or without food.
* If a dose is missed, take it as soon as remembered, unless it is almost time for the next dose. Do not double doses.
* Due to the potential for drug interactions, especially with clarithromycin, a thorough medication history is crucial.
