# Sompraz HP
## Overview
Sompraz HP is a combination product containing pantoprazole (a proton pump inhibitor), amoxicillin (a penicillin-class antibiotic), and clarithromycin (a macrolide antibiotic). It is used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
The typical adult dose is one Sompraz HP capsule taken twice daily for 7 to 14 days, as prescribed. The capsule contains pantoprazole 40 mg, amoxicillin 500 mg, and clarithromycin 500 mg.
## Pediatric Dosing
Dosing for pediatric patients is not established and should be determined by a specialist based on weight and clinical judgment.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment for pantoprazole is recommended in mild to moderate renal impairment. In severe renal impairment, caution may be advised. For amoxicillin, dose reduction may be necessary in severe renal impairment. Clarithromycin dose adjustment is needed in renal impairment.
* **Hepatic Impairment:** Pantoprazole dose reduction may be necessary in severe hepatic impairment. Clarithromycin dose adjustment is recommended in hepatic impairment.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, other macrolides, penicillins, or any component of the formulation.
* History of cholestatic jaundice or hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with ergotamine, dihydroergotamine, cisapride, pimozide, or lovastatin/simvastatin.
## Adverse Effects
Common adverse effects include diarrhea, nausea, headache, abdominal pain, taste perversion, rash, and changes in liver enzymes. Serious adverse effects can include *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions, *Stevens-Johnson syndrome/toxic epidermal necrolysis*, QT prolongation, and tendon rupture.
## Key Drug Interactions
* **Pantoprazole:** May decrease the absorption of drugs requiring gastric acidity (e.g., ketoconazole, itraconazole, ritonavir, amlodipine, warfarin). May interact with methotrexate.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. Probenecid may increase amoxicillin levels.
* **Clarithromycin:** Potent inhibitor of CYP3A4. Significant interactions with warfarin, theophylline, carbamazepine, cyclosporine, digoxin, ergot alkaloids, statins (lovastatin, simvastatin), sildenafil, and other CYP3A4 substrates. Increases plasma concentrations of colchicine.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for *Clostridioides difficile*-associated diarrhea.
* Assess for improvement of ulcer symptoms and confirmation of *H. pylori* eradication (e.g., urea breath test, stool antigen test).
* Monitor liver function tests, especially with prolonged use or in patients with hepatic impairment.
## Clinical Pearls
* Administer the entire contents of the capsule (pantoprazole, amoxicillin, and clarithromycin) twice daily.
* Dosing duration is typically 7 to 14 days, but may vary based on local resistance patterns and patient-specific factors.
* *H. pylori* eradication rates can be affected by antibiotic resistance; consider local susceptibility data.
* Advise patients to complete the full course of therapy even if symptoms improve.
