# Sompraz HP
## Overview
Sompraz HP is a combination product containing dexlansoprazole (a proton pump inhibitor) and amoxicillin and clarithromycin (antibiotics). It is used for the eradication of *Helicobacter pylori* (*H. pylori*).
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
The standard adult dose is one Sompraz HP unit taken orally twice daily for 10 to 14 days. Each unit typically contains dexlansoprazole 60 mg, amoxicillin 1 gram (500 mg twice daily), and clarithromycin 500 mg (250 mg twice daily). The exact composition and dosing schedule may vary; confirm with product labeling.
## Pediatric Dosing
Pediatric dosing for Sompraz HP is not specifically established. Dosing for *H. pylori* eradication in children is individualized based on age, weight, and local resistance patterns, often utilizing separate amoxicillin, clarithromycin, and a proton pump inhibitor.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment for the combination product, but individual components may have specific recommendations.
## Contraindications
Hypersensitivity to dexlansoprazole, amoxicillin, clarithromycin, or other macrolides. History of cholestatic jaundice or hepatic dysfunction associated with prior clarithromycin use. Concurrent use with cisapride, pimozide, or other drugs metabolized by CYP3A4 with narrow therapeutic indices.
## Adverse Effects
Common adverse effects include diarrhea, nausea, headache, abdominal pain, and dysgeusia (altered taste). Clarithromycin can cause QT prolongation, and amoxicillin can cause rash or hypersensitivity reactions.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Clarithromycin is a moderate inhibitor of CYP3A4. Co-administration with drugs metabolized by CYP3A4 (e.g., warfarin, digoxin, certain statins, benzodiazepines) can increase their serum concentrations and risk of toxicity.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
* **Dexlansoprazole:** May affect the absorption of other drugs requiring gastric acidity for absorption (e.g., ketoconazole, iron salts).
## Monitoring
* Assess for signs and symptoms of *H. pylori* infection resolution (e.g., relief of ulcer symptoms).
* Monitor for adverse effects, particularly diarrhea, rash, and signs of liver dysfunction.
* If using concurrently with narrow therapeutic index CYP3A4 substrates, monitor for signs of toxicity.
## Clinical Pearls
* Administer the dose as prescribed for the full duration, even if symptoms improve, to ensure eradication.
* Ensure patients understand the importance of taking both doses daily.
* Consider local *H. pylori* resistance patterns when selecting treatment.
* Advise patients to report severe diarrhea, rash, or signs of allergic reaction immediately.
