# Sompraz HP
## Overview
Sompraz HP is a fixed-dose combination product containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* (*H. pylori*). Pantoprazole is a proton pump inhibitor (PPI), and amoxicillin and clarithromycin are antibiotics.
## Primary Indications
* Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
The standard adult regimen is one Sompraz HP pack taken twice daily for 7 to 14 days. Each pack typically contains:
* Pantoprazole 40 mg
* Amoxicillin 1000 mg (given as two 500 mg capsules)
* Clarithromycin 500 mg
**Note:** Specific pack contents and duration may vary based on local prescribing information and patient factors.
## Pediatric Dosing
Dosing for Sompraz HP in pediatric patients is not established. Dosing of individual components may be used, but requires careful consideration of age, weight, and renal/hepatic function.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is typically needed for pantoprazole or clarithromycin, although caution is advised with severe renal impairment. Amoxicillin dose adjustment may be necessary in severe renal impairment (CrCl < 30 mL/min).
* **Hepatic Impairment:** No dose adjustment is typically needed for pantoprazole. Clarithromycin dose adjustment is recommended in patients with hepatic impairment and/or renal impairment. Amoxicillin dose adjustment is usually not necessary.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, other macrolides, penicillins, or any component of the formulation.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with certain drugs, such as rifabutin, cisapride, pimozide, and ergotamine derivatives (due to clarithromycin).
* Severe hepatic impairment.
## Adverse Effects
Common adverse effects include:
* **Gastrointestinal:** Diarrhea, nausea, abdominal pain, vomiting, constipation, taste disturbances.
* **Other:** Headache, rash, dizziness, rash.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions, severe skin reactions (e.g., Stevens-Johnson syndrome), hepatic dysfunction, QT prolongation (clarithromycin).
## Key Drug Interactions
* **Pantoprazole:** Can affect absorption of drugs dependent on gastric pH (e.g., ketoconazole, itraconazole, clopidogrel).
* **Clarithromycin:** Potent inhibitor of CYP3A4, leading to increased concentrations of many drugs metabolized by this enzyme (e.g., statins, warfarin, some antiretrovirals, certain immunosuppressants). Can also interact with other CYP inhibitors/inducers.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. Probenecid may increase and prolong blood levels of amoxicillin.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Assess for diarrhea and rule out *C. difficile*-associated diarrhea.
* Evaluate for signs of hepatotoxicity.
* Monitor for drug interactions, especially with clarithromycin.
* A follow-up urea breath test or stool antigen test is recommended 4-6 weeks after treatment completion to confirm *H. pylori* eradication.
## Clinical Pearls
* Sompraz HP should be taken as prescribed for the full duration to ensure *H. pylori* eradication and prevent resistance.
* Take pantoprazole 30-60 minutes before the morning meal. The antibiotics can be taken with or without food.
* If a dose is missed, take it as soon as remembered, unless it is close to the time for the next dose. Do not double doses.
* Patients with penicillin allergy should not receive amoxicillin.
