# Sompraz HP
## Overview
Sompraz HP is a combination product containing esomeprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
* Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
The typical adult regimen is:
* Esomeprazole 20 mg orally twice daily
* Amoxicillin 1 gram orally twice daily
* Clarithromycin 500 mg orally twice daily
* This regimen is usually prescribed for 10 to 14 days.
## Pediatric Dosing
There is no established pediatric dosing for Sompraz HP. Pediatric use is generally not recommended due to lack of data. Dosing for individual components in pediatric patients would need to be calculated based on weight and specific indications, following guideline recommendations and consulting with pediatric specialists.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment for esomeprazole is generally required. However, amoxicillin and clarithromycin doses may need adjustment based on the severity of renal impairment, following individual component prescribing information.
* **Hepatic Impairment:** For esomeprazole, caution is advised; a maximum dose of 20 mg daily may be appropriate. Amoxicillin and clarithromycin dose adjustments are generally not required in mild to moderate hepatic impairment but should be used with caution.
## Contraindications
* Hypersensitivity to esomeprazole, amoxicillin, clarithromycin, or other macrolides or penicillins.
* History of hypersensitivity reactions to amoxicillin, penicillin, or cephalosporins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with ticagrelor or ranolazine (due to clarithromycin).

## Adverse Effects
Common adverse effects include diarrhea, nausea, vomiting, abdominal pain, taste disturbances, headache, rash, and potential for *Clostridioides difficile*-associated diarrhea. Serious adverse effects can include severe hypersensitivity reactions, QT prolongation (clarithromycin), and hepatic dysfunction.
## Key Drug Interactions
* **Esomeprazole:** May affect absorption of other drugs (e.g., ketoconazole, iron salts). Can increase levels of certain drugs metabolized by CYP2C19 or CYP3A4.
* **Clarithromycin:** Potent inhibitor of CYP3A4, can increase serum concentrations of numerous drugs (e.g., simvastatin, lovastatin, amlodipine, carbamazepine, cyclosporine, tacrolimus, sildenafil). Interacts with colchicine, leading to potential toxicity. Avoids use with ticagrelor and ranolazine.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Assess for diarrhea, which could indicate *C. difficile*-associated diarrhea.
* Monitor liver function tests, especially with prolonged use or in patients with pre-existing hepatic disease.
* Assess for therapeutic efficacy (eradication of *H. pylori*) via appropriate testing (e.g., urea breath test, stool antigen test) after completion of therapy.
## Clinical Pearls
* Ensure patients complete the full course of therapy as prescribed, even if symptoms improve.
* Administer esomeprazole 30-60 minutes before meals for optimal acid suppression.
* Advise patients to report any severe diarrhea, rash, or signs of allergic reaction immediately.
* Consider resistance patterns for clarithromycin in the local area when initiating therapy.
