# Sompraz HP
## Overview
Sompraz HP is a combination product containing the proton pump inhibitor (PPI) omeprazole, the antibiotics amoxicillin, and clarithromycin. It is indicated for the eradication of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Primary Indications
* Eradication of *Helicobacter pylori* in patients with duodenal ulcer disease or gastric ulcer disease.
## Adult Dosing
**Sompraz HP** is typically dosed as one capsule twice daily for 10 to 14 days.
Each capsule contains:
* Omeprazole 20 mg
* Amoxicillin 500 mg
* Clarithromycin 250 mg
**Regimen:** Take one capsule in the morning and one capsule in the evening for 10 to 14 days.
## Pediatric Dosing
There is no established pediatric dosing for Sompraz HP. Dosing for pediatric patients should be based on individual components and guided by expert recommendation.
## Dose Adjustments
No specific dose adjustments are required for renal or hepatic impairment for the combination product. However, individual components may require adjustment.
* **Amoxicillin:** Dose reduction may be necessary in severe renal impairment.
* **Clarithromycin:** Dose adjustment is recommended in patients with moderate to severe renal impairment (CrCl < 30 mL/min).
* **Omeprazole:** No dose adjustment is generally needed for renal or hepatic impairment, though caution may be advised in severe hepatic disease.
## Contraindications
* Hypersensitivity to omeprazole, amoxicillin, clarithromycin, or any component of the formulation.
* History of allergic reactions to penicillins or cephalosporins.
* Concomitant use with ergot derivatives, cisapride, pimozide, simvastatin, or lovastatin due to risk of serious adverse events.
* Patients with a history of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Severe hepatic impairment (for clarithromycin component, although omeprazole may also require caution).
* Electrolyte disturbances.
## Adverse Effects
Common adverse effects include diarrhea, nausea, vomiting, abdominal pain, taste disturbances, headache, and rash.
More serious adverse effects can include:
* *Clostridioides difficile*-associated diarrhea (CDAD)
* Hypersensitivity reactions (including anaphylaxis)
* Hepatotoxicity
* Arrhythmias (with clarithromycin, particularly in patients with risk factors)
* Neurological effects (e.g., dizziness, confusion)
* Renal impairment
## Key Drug Interactions
* **Omeprazole:** Can affect the absorption of other drugs by altering gastric pH. May increase serum concentrations of drugs metabolized by CYP2C19 (e.g., clopidogrel, voriconazole) and CYP3A4 (e.g., diazepam, warfarin).
* **Clarithromycin:** A potent inhibitor of CYP3A4. Can significantly increase serum concentrations of drugs metabolized by CYP3A4 (e.g., statins, calcium channel blockers, benzodiazepines, anticoagulants like warfarin). Avoid concomitant use with colchicine, ergotamines, cisapride, pimozide, simvastatin, and lovastatin. May increase the effect of warfarin.
* **Amoxicillin:** May decrease the effectiveness of oral contraceptives. Probenecid may decrease the renal excretion of amoxicillin.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Assess for development of diarrhea, particularly *C. difficile*-associated diarrhea.
* Monitor renal and hepatic function, especially in patients with pre-existing impairment or those receiving prolonged therapy.
* Monitor for effectiveness of *H. pylori* eradication, typically by urea breath test or stool antigen test, at least 4 weeks after completion of therapy.
## Clinical Pearls
* This combination therapy is a standard regimen for *H. pylori* eradication.
* Compliance with the full 10-14 day course is crucial for successful eradication.
* Patients should be advised to take the medication as prescribed and to report any severe or persistent adverse effects.
* Ensure adequate hydration and electrolyte balance, especially if significant diarrhea occurs.
* Consider potential drug interactions, particularly with clarithromycin's CYP3A4 inhibition.
