# Sompraz HP
## Overview
Sompraz HP is a combination medication containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
Treatment of *H. pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
The standard adult dose is one capsule (containing pantoprazole 40 mg, amoxicillin 500 mg, and clarithromycin 500 mg) twice daily for 7 to 14 days.
## Pediatric Dosing
Pediatric dosing is not established for this combination product. Dosing for individual components may vary and should be determined by a pediatrician based on weight and clinical status.
## Dose Adjustments
No specific dose adjustments are recommended for renal or hepatic impairment for this combination product. ## Contraindications

Serious adverse effects can include severe allergic reactions (including anaphylaxis), *Clostridioides difficile*-associated diarrhea, liver dysfunction, QT prolongation, and potential development of antimicrobial resistance.
## Key Drug Interactions
* **Pantoprazole:** May decrease the absorption of drugs requiring an acidic gastric pH (e.g., ketoconazole, itraconazole, certain HIV protease inhibitors). May interact with warfarin.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. May interact with warfarin.
* **Clarithromycin:** Potent inhibitor of CYP3A4. Significant interactions are possible with drugs metabolized by CYP3A4, including but not limited to: colchicine, certain statins (atorvastatin, lovastatin), certain calcium channel blockers, certain anticoagulants (e.g., rivaroxaban), theophylline, and sildenafil. May increase serum concentrations of digoxin. Avoid use with ergots.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* Assess for improvement in ulcer symptoms and *H. pylori* eradication (typically via urea breath test, stool antigen test, or biopsy 4-6 weeks after completion of therapy).
* Monitor for diarrhea, especially *C. difficile*-associated diarrhea.
* Monitor liver function tests, particularly with clarithromycin.
* Assess for potential drug interactions based on concomitant medications.
## Clinical Pearls
* This combination therapy is designed for a specific duration to maximize *H. pylori* eradication rates and minimize resistance development.
* Ensure patients complete the full course of therapy as prescribed.
* Advise patients to take the medication as directed, typically with food.
* If a dose is missed, advise patients to take it as soon as remembered, but not to double the dose.
