# Sompraz HP
## Overview
Sompraz HP is a combination product containing lansoprazole, amoxicillin, and clarithromycin, indicated for the eradication of *Helicobacter pylori*.
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
The standard adult dose is one kit taken twice daily for 14 days. Each kit contains:
* Lansoprazole 30 mg capsule
* Amoxicillin 500 mg capsule
* Clarithromycin 500 mg capsule
* Take one of each capsule twice daily (morning and evening) for 14 days.
* Patients should be instructed to take the medication as directed for the full 14-day course.
## Pediatric Dosing
Dosing for pediatric patients has not been established for this specific combination product. Dosing for individual components may be considered based on age and weight, but requires careful consideration and consultation with a pediatrician.
## Dose Adjustments
No dose adjustments are routinely recommended for patients with renal or hepatic impairment for this specific combination product. However, caution may be advised in severe impairment.
## Contraindications

## Key Drug Interactions
* **Lansoprazole:** Can increase levels of drugs metabolized by CYP2C19 (e.g., clopidogrel, phenytoin) and CYP3A4. Avoid concomitant use with rilpivirine.
* **Clarithromycin:** A potent CYP3A4 inhibitor. Can increase levels of drugs metabolized by CYP3A4 (e.g., statins, warfarin, benzodiazepines, certain calcium channel blockers, colchicine). May interact with other CYP3A4 inhibitors or inducers. Avoid concomitant use with colchicine.
* **Amoxicillin:** May decrease the effectiveness of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for signs and symptoms of *Clostridioides difficile*-associated diarrhea.
* Monitor liver function tests if there is a history of liver disease or if signs/symptoms of hepatotoxicity develop.
* Assess for improvement in ulcer symptoms and confirmation of *H. pylori* eradication (e.g., urea breath test, stool antigen test) after treatment completion, as per local guidelines.
## Clinical Pearls
* This combination therapy is intended for a short duration (14 days) for *H. pylori* eradication.
* It is crucial for patients to complete the entire 14-day course of therapy, even if they feel better, to ensure successful eradication.
* Patients should be advised to report any severe diarrhea, rash, or signs of allergic reaction immediately.
* Consider the potential for antibiotic resistance when prescribing *H. pylori* eradication regimens.
