# Sompraz HP (Omeprazole and Sodium Bicarbonate)
## Overview
Sompraz HP is a combination product containing omeprazole, a proton pump inhibitor (PPI), and sodium bicarbonate, an antacid. Omeprazole reduces gastric acid production, while sodium bicarbonate provides rapid, albeit short-lived, neutralization of stomach acid. This combination is designed for the treatment of duodenal ulcers associated with *Helicobacter pylori* infection.
## Primary Indications
* Treatment of active duodenal ulcers.
* Eradication of *Helicobacter pylori* infection in patients with duodenal ulcers, in combination with appropriate antibiotics.
## Adult Dosing
The typical adult regimen for *H. pylori* eradication involves Sompraz HP in conjunction with two antibiotics (e.g., amoxicillin and clarithromycin, or metronidazole and clarithromycin).
**Standard Regimen:**
* One capsule (containing omeprazole 20 mg and sodium bicarbonate 1100 mg) orally once daily for 14 days.
**Note:** The specific antibiotic regimen and duration should follow local or national guidelines.
## Pediatric Dosing
Specific pediatric dosing for Sompraz HP is not established. Use in pediatric populations should be based on expert recommendation and careful consideration of the risks and benefits.
## Dose Adjustments
* **Hepatic Impairment:** No specific dose adjustment is recommended for mild to moderate hepatic impairment. However, caution is advised. For severe hepatic impairment, caution is warranted, and dose reduction may be considered based on clinical response.
* **Renal Impairment:** No dose adjustment is generally required.
## Contraindications
* Known hypersensitivity to omeprazole, sodium bicarbonate, or any component of the formulation.
* Concomitant use with ritonavir.
* Patients with a history of hypersensitivity reactions to other proton pump inhibitors.
* Patients with a history of allergies to bicarbonate-containing products.
* Conditions where excessive bicarbonate levels could be harmful (e.g., severe renal impairment, congestive heart failure, hypertension, edema).
## Adverse Effects
Common adverse effects are generally related to the gastrointestinal system and may include:
* Diarrhea
* Nausea
* Vomiting
* Abdominal pain
* Headache
* Flatulence
* Constipation
* Dizziness
Serious adverse effects, though less common, can include:
* *Clostridium difficile*-associated diarrhea
* Bone fractures (hip, wrist, spine) with prolonged use
* Hypomagnesemia
* Vitamin B12 deficiency with prolonged use
* Acute tubulointerstitial nephritis
## Key Drug Interactions
* **Drugs Affected by Gastric pH:** Omeprazole can alter the absorption of drugs whose absorption is dependent on gastric pH. Examples include ketoconazole, itraconazole, posaconazole, and certain HIV protease inhibitors (e.g., atazanavir, nelfinavir).
* **CYP2C19 Substrates:** Omeprazole is an inhibitor of CYP2C19 and can increase plasma concentrations of drugs metabolized by this enzyme (e.g., clopidogrel, citalopram, sertraline).
* **Methotrexate:** PPIs may increase methotrexate levels, potentially leading to toxicity.
* **Voriconazole:** May increase omeprazole levels.
* **Rifampin:** May decrease omeprazole levels.
* **Warfarin:** Increased INR and prothrombin time have been observed with concomitant use.
* **Digoxin:** Concomitant use may increase digoxin levels due to omeprazole's effect on gastric pH.
## Monitoring
* Monitor for signs and symptoms of *H. pylori* eradication failure.
* Monitor for signs and symptoms of adverse effects, especially with prolonged therapy.
* Consider monitoring serum magnesium levels in patients on long-term therapy or those taking concomitant medications that may cause hypomagnesemia (e.g., diuretics).
* Assess for potential drug interactions, especially with concomitant medications.
## Clinical Pearls
* Sompraz HP provides rapid symptom relief due to the sodium bicarbonate component, followed by sustained acid suppression from omeprazole.
* The entire contents of the capsule should be taken as a single dose to ensure adequate omeprazole delivery and efficacy. Do not chew or crush the pellets within the capsule.
* This combination is specifically designed for *H. pylori* eradication regimens and should be used as part of a multi-drug therapy.
* Long-term use of PPIs is associated with potential risks, including bone fractures and vitamin B12 deficiency.
