# Sompraz HP (Pantoprazole HP)
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion. "HP" likely indicates a high-potency formulation, but specific characteristics require clarification.
## Primary Indications
* Gastroesophageal reflux disease (GERD)
* Erosive esophagitis
* Healing of duodenal ulcers
* Zollinger-Ellison syndrome
## Adult Dosing
* **GERD/Erosive Esophagitis:** 40 mg orally once daily for up to 8 weeks.
* **Duodenal Ulcers:** 40 mg orally once daily for up to 4 weeks.
* **Zollinger-Ellison Syndrome:** Dosing is highly individualized. Initiate with 40 mg orally twice daily. Doses may need to be increased. Maximum recommended dose is 240 mg/day.
## Pediatric Dosing
Dosing is not well-established for pantoprazole HP in pediatrics. Standard pantoprazole dosing for children 5 years and older:
* **GERD/Erosive Esophagitis:** 20-40 mg orally once daily for up to 8 weeks.
## Dose Adjustments
* **Hepatic Impairment:** Reduce daily dose to 20 mg.
* **Renal Impairment:** No dose adjustment necessary.
## Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
## Adverse Effects
* **Common:** Diarrhea, headache, nausea, dizziness, abdominal pain.
* **Serious:** *Clostridioides difficile*-associated diarrhea, bone fractures (hip, wrist, spine) with long-term use, hypomagnesemia, vitamin B12 deficiency.
## Key Drug Interactions
* **Antiretrovirals:** May decrease absorption of atazanavir, nelfinavir.
* **Methotrexate:** May increase methotrexate levels.
* **CYP2C19 Substrates:** May affect metabolism of drugs like clopidogrel.
## Monitoring
* Monitor for signs and symptoms of *C. difficile* infection.
* Consider monitoring magnesium levels with prolonged therapy (>1 year).
* Assess for potential bone fracture risk with long-term use.
* For Zollinger-Ellison syndrome, monitor gastric pH.
## Clinical Pearls
* Administer at least 30 minutes before a meal.
* Swallow capsules whole; do not crush or chew.
* Long-term PPI use is associated with increased risk of fractures, hypomagnesemia, and vitamin B12 deficiency.
* The "HP" designation requires clarification regarding its specific meaning and any associated dosing implications.
