### Overview

### Primary Indications
* Gastroesophageal reflux disease (GERD)
* Erosive esophagitis
* Zollinger-Ellison syndrome
* Healing of duodenal ulcers
### Adult Dosing
* **GERD/Erosive Esophagitis:** 40 mg once daily, may increase to 40 mg twice daily for severe cases. Duration of treatment varies.
* **Healing of Duodenal Ulcers:** 40 mg once daily.
* **Zollinger-Ellison Syndrome:** Starting dose typically 40 mg twice daily, may be adjusted based on gastric acid output. Doses up to 160 mg daily have been used.
### Pediatric Dosing
Dosing varies significantly by age and indication. Consult specific pediatric guidelines.
* **12 years and older:**
* **GERD/Erosive Esophagitis:** 40 mg once daily for up to 8 weeks.
### Dose Adjustments
No dose adjustment is typically required for hepatic or renal impairment.
### Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
### Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, and dizziness. Long-term PPI use may be associated with an increased risk of fractures (hip, wrist, spine), C. difficile infection, hypomagnesemia, and vitamin B12 deficiency.
### Key Drug Interactions
* **Clopidogrel:** Pantoprazole may decrease the antiplatelet effect of clopidogrel. Consider alternative PPIs or separate administration times if concomitant use is necessary.
* **Ketoconazole, Itraconazole, Posaconazole:** Gastric pH is increased, potentially decreasing absorption of these antifungals.
* **Methotrexate:** PPIs may increase methotrexate levels, potentially leading to toxicity.
### Monitoring
* Monitor for signs and symptoms of C. difficile infection.
* Consider periodic serum magnesium monitoring, especially with prolonged therapy or concomitant diuretic use.
* Monitor for signs of bone fracture.
* Assess for vitamin B12 deficiency symptoms.
### Clinical Pearls
* Administer pantoprazole 30 minutes before a meal for optimal efficacy.
* Do not crush or chew delayed-release formulations.
* Tapering may be considered when discontinuing long-term therapy to minimize rebound acid hypersecretion.
