# Sompraz HP
## Overview
Sompraz HP is a combination product containing omeprazole, amoxicillin, and clarithromycin. It is used for the eradication of *Helicobacter pylori* (*H. pylori*).
## Primary Indications
Treatment of *Helicobacter pylori* infection in patients with duodenal ulcer disease.
## Adult Dosing
The standard adult dose is one kit (containing 40 mg omeprazole, 500 mg amoxicillin, and 500 mg clarithromycin) twice daily for 10-14 days.
## Pediatric Dosing
There is no established pediatric dosing for Sompraz HP. Dosing for *H. pylori* eradication in children is individualized based on weight and local guidelines.
## Dose Adjustments
No dose adjustments are typically needed for renal or hepatic impairment, although caution is advised in severe hepatic dysfunction.
## Contraindications
* Hypersensitivity to omeprazole, amoxicillin, clarithromycin, or other macrolides or penicillins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with cisapride, dihydroergotamine, ergotamine, or pimozide.
* Severe renal impairment (clarithromycin component).
## Adverse Effects
Common adverse effects include diarrhea, nausea, vomiting, abdominal pain, taste disturbances, headache, and rash. Clarithromycin can cause QT prolongation and potential cardiac events. Amoxicillin can cause hypersensitivity reactions.
## Key Drug Interactions
* **Omeprazole:** May decrease the absorption of drugs requiring gastric acidity (e.g., ketoconazole, itraconazole, ritonavir). May increase levels of CYP2C19 substrates (e.g., clopidogrel, phenytoin).
* **Clarithromycin:** Potent inhibitor of CYP3A4. Increases levels of numerous drugs including statins, benzodiazepines, carbamazepine, colchicine, cyclosporine, digoxin, and warfarin. Contraindicated with cisapride, ergotamine derivatives, pimozide, and lovastatin/simvastatin.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for signs and symptoms of *C. difficile*-associated diarrhea.
* Monitor electrolytes and QT interval with clarithromycin, especially in patients with risk factors.
* Confirmation of *H. pylori* eradication is typically done by stool antigen testing or urea breath test 4-6 weeks after completion of therapy.
## Clinical Pearls
* Administer the dose in the morning and evening.
* Take omeprazole before a meal.
* Ensure completion of the full course of therapy, even if symptoms improve.
* Advise patients to report any severe diarrhea or signs of allergic reaction immediately.
