# Sompraz HP
## Overview
Sompraz HP is a combination medication containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori*.
## Primary Indications
* Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
* **Sompraz HP:** One packet twice daily for 7 days. Each packet contains pantoprazole 40 mg, amoxicillin 500 mg, and clarithromycin 500 mg.
## Pediatric Dosing
* Dosing for pediatric patients is not established.
## Dose Adjustments
* **Renal Impairment:** Amoxicillin and clarithromycin require dose adjustment. Pantoprazole dose generally does not require adjustment. Due to the combination product, separate agents may be preferred in significant renal impairment.
* **Hepatic Impairment:** Clarithromycin requires dose reduction. Pantoprazole dose generally does not require adjustment. Due to the combination product, separate agents may be preferred in significant hepatic impairment.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, other macrolides, or penicillins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with ergotamine or dihydroergotamine.
* Severe renal impairment with creatinine clearance < 30 mL/min (for clarithromycin component, though specific data for combination product is limited).
## Adverse Effects
* **Common:** Diarrhea, nausea, abdominal pain, vomiting, taste disturbance, headache, rash.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions, Stevens-Johnson syndrome, toxic epidermal necrolysis, hepatotoxicity (clarithromycin), QT prolongation (clarithromycin), potential for amoxicillin-induced rash in patients with mononucleosis.
## Key Drug Interactions
* **Clarithromycin:** Potent CYP3A4 inhibitor. Interactions with statins, warfarin, digoxin, certain antiarrhythmics, benzodiazepines, calcium channel blockers, and many others. Avoid coadministration with ergot derivatives.
* **Pantoprazole:** Can affect absorption of drugs requiring acidic environment (e.g., ketoconazole, itraconazole, certain protease inhibitors). May decrease the efficacy of clopidogrel.
* **Amoxicillin:** May decrease efficacy of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for *Clostridioides difficile*-associated diarrhea.
* Monitor liver function tests, especially with prolonged use or in patients with pre-existing liver disease.
* Assess *H. pylori* eradication status post-treatment (e.g., urea breath test, stool antigen test).
## Clinical Pearls
* Take 1 packet in the morning and 1 packet in the evening for 7 days.
* Administer pantoprazole 30 minutes before meals. The combination packet should be taken as directed by the packet instructions (typically before breakfast and dinner).
* Ensure patient is not allergic to penicillins or macrolides.
* Advise patients to complete the full course of therapy, even if they feel better.
* Consider the potential for antibiotic resistance in *H. pylori*.
