# Sompraz hp
## Overview
Sompraz hp is a fixed-dose combination product containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection. Pantoprazole is a proton pump inhibitor (PPI), while amoxicillin and clarithromycin are antibiotics.
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
The typical adult dose is one kit taken twice daily for 10 days. Each kit contains:
* Pantoprazole 40 mg delayed-release tablet
* Amoxicillin 500 mg capsule
* Clarithromycin 500 mg tablet
**Important:** Dosing may vary based on local resistance patterns and clinical guidelines.
## Pediatric Dosing
*Helicobacter pylori* eradication is not typically recommended in children under 12 years of age. Dosing in pediatric patients over 12 years old should be based on adult recommendations, but specific pediatric guidelines should be consulted.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is recommended for pantoprazole in mild to moderate renal impairment. Caution is advised in severe renal impairment. Amoxicillin and clarithromycin require dose adjustments in significant renal impairment; consult specific drug information.
* **Hepatic Impairment:** No dose adjustment is recommended for pantoprazole in mild to moderate hepatic impairment. Caution is advised in severe hepatic impairment. Clarithromycin may require dose adjustment in significant hepatic impairment; consult specific drug information.
## Contraindications
* Known hypersensitivity to pantoprazole, amoxicillin, clarithromycin, other macrolides, or penicillins.

* Concomitant use with rifampin, astemizole, cisapride, ergotamine, ergotamine tartrate, mesoridazine, pimozide, quinidine, or terfenadine.
## Adverse Effects
Common adverse effects include diarrhea, headache, nausea, abdominal pain, vomiting, dizziness, rash, and altered taste. More serious potential adverse effects include severe allergic reactions, *Clostridioides difficile*-associated diarrhea, QT prolongation (especially with clarithromycin), and hepatotoxicity.
## Key Drug Interactions
* **Pantoprazole:** May decrease the absorption of drugs requiring gastric acidity for absorption (e.g., ketoconazole, itraconazole, atazanavir). May affect CYP2C19 metabolism of other drugs (e.g., clopidogrel, warfarin).
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
* **Clarithromycin:** Potent inhibitor of CYP3A4. May increase serum concentrations of various drugs metabolized by CYP3A4 (e.g., statins, calcium channel blockers, benzodiazepines, theophylline, warfarin). Increased risk of myopathy and rhabdomyolysis with statins.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* Assess for effectiveness of *H. pylori* eradication (e.g., urea breath test, stool antigen test) at least 4 weeks after treatment completion.
* Monitor for diarrhea, especially *C. difficile*-associated diarrhea.
* Monitor liver function tests if hepatic impairment is a concern.
## Clinical Pearls
* Administer the kit as directed, usually with a meal and before bedtime.
* Patients should complete the full course of therapy, even if symptoms improve.
* Due to high resistance rates of *H. pylori* to clarithromycin in some regions, this regimen's efficacy may be reduced. Local susceptibility data should guide therapy.
* Advise patients to report any severe diarrhea, skin rash, or signs of liver problems immediately.
