# Sompraz hp
## Overview
Sompraz hp is a combination medication containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection. Pantoprazole is a proton pump inhibitor (PPI), while amoxicillin and clarithromycin are antibiotics.
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
The standard dose is one triple-therapy pack taken twice daily for 7 to 14 days. Each pack typically contains:
* Pantoprazole 40 mg delayed-release tablet
* Amoxicillin 500 mg capsule
* Clarithromycin 500 mg tablet
Patients should take all components of the pack twice daily before a meal. The duration of therapy (7-14 days) should be determined by the prescribing physician.
## Pediatric Dosing
Specific pediatric dosing for Sompraz hp is not established. Dosing for individual components would need to be calculated based on weight and indication, and may involve separate formulations.
## Dose Adjustments
No dose adjustments are typically required for patients with renal or hepatic impairment, though caution may be advised with severe hepatic impairment for pantoprazole.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or other macrolides or penicillins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Use in patients with known heart rhythm abnormalities or QT prolongation.
* Concomitant use with cisapride, pimozide, or ergotamine derivatives.
## Adverse Effects
Common adverse effects include diarrhea, nausea, headache, and abdominal pain. Other potential side effects include rash, dizziness, and taste disturbances.
## Key Drug Interactions
* **Clarithromycin:** Can inhibit CYP3A4, increasing levels of drugs metabolized by this enzyme (e.g., warfarin, certain statins, certain calcium channel blockers, cyclosporine, tacrolimus, digoxin, some benzodiazepines).
* **Pantoprazole:** Can alter gastric pH, affecting the absorption of certain drugs (e.g., ketoconazole, itraconazole, certain HIV protease inhibitors). May increase INR in patients on warfarin.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of allergic reactions.
* Assess for improvement in ulcer symptoms.
* Monitor for *C. difficile*-associated diarrhea.
* Monitor INR in patients on warfarin.
## Clinical Pearls
* Ensure patient adherence to the full course of therapy.
* Instruct patients to take the medication as directed, typically before meals.
* If amoxicillin allergy is present, alternative *H. pylori* eradication regimens are necessary.
* Clarify the specific duration of therapy based on current guidelines and patient factors.
