# Sompraz HP
## Overview
Sompraz HP is a combination product containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
**Standard Regimen:**
* Pantoprazole 40 mg orally once daily
* Amoxicillin 1000 mg orally twice daily
* Clarithromycin 500 mg orally twice daily
**Duration:** 7 to 14 days.
## Pediatric Dosing
No established pediatric dosing for this combination product. Dosing of individual components would need to be determined by a prescriber based on weight and clinical assessment.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment recommended for pantoprazole in mild to moderate renal impairment. Use with caution in severe renal impairment. Amoxicillin and clarithromycin require dose adjustment in significant renal impairment.
* **Hepatic Impairment:** No specific dose adjustment recommended for pantoprazole in mild to moderate hepatic impairment. Use with caution in severe hepatic impairment.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or other macrolides or penicillins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with ergotamine derivatives.
* Patients with known hypersensitivity to penicillin.
* Patients with a history of *Clostridium difficile*-associated diarrhea.
## Adverse Effects
* **Common:** Diarrhea, nausea, abdominal pain, taste disturbance, headache, rash.
* **Serious:** *Clostridium difficile*-associated diarrhea, severe allergic reactions (anaphylaxis), Stevens-Johnson syndrome, toxic epidermal necrolysis, QT prolongation (clarithromycin), hepatic dysfunction, rhabdomyolysis (statins).
## Key Drug Interactions
* **Pantoprazole:** Decreases absorption of drugs requiring acidic gastric pH (e.g., ketoconazole, itraconazole, certain HIV protease inhibitors). May increase levels of drugs metabolized by CYP2C19 (e.g., clopidogrel).
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. Probenecid decreases renal excretion of amoxicillin.
* **Clarithromycin:** Potent CYP3A4 inhibitor. Increases serum concentrations of numerous drugs, including statins (risk of myopathy/rhabdomyolysis), warfarin, digoxin, carbamazepine, theophylline, and certain benzodiazepines. Contraindicated with colchicine, ergotamines.
## Monitoring
* Signs and symptoms of *H. pylori* eradication (e.g., resolution of dyspepsia).
* Signs and symptoms of hypersensitivity reactions.
* Signs of *Clostridium difficile*-associated diarrhea.
* Liver function tests may be considered with prolonged use or in patients with pre-existing liver disease.
## Clinical Pearls
* This combination therapy is intended for short-term use.
* Administer pantoprazole 30-60 minutes before a meal.
* Ensure the patient completes the full course of antibiotics.
* Consider local antibiotic resistance patterns when selecting therapy.
* Advise patients to report severe or persistent diarrhea immediately.
