# Sompraz hp
## Overview
Sompraz hp is a combination product containing lansoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori*. Lansoprazole is a proton pump inhibitor (PPI), while amoxicillin and clarithromycin are antibiotics.
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease or duodenal ulcer disease.
## Adult Dosing
The typical adult dose is one (1) packet taken twice daily for 10 to 14 days. Each packet contains lansoprazole 30 mg, amoxicillin 1 g, and clarithromycin 500 mg. The contents of the packet should be mixed with water and consumed immediately.
## Pediatric Dosing
Specific pediatric dosing for Sompraz hp is not established. Dosing for *H. pylori* eradication in children typically involves weight-based calculations for each individual component and should be determined by a pediatrician or infectious disease specialist.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment with this specific combination product. However, individual components may require adjustments in severe renal impairment. Consult specific drug monographs for amoxicillin and clarithromycin for guidance.
## Contraindications
* Hypersensitivity to any component of the formulation (lansoprazole, amoxicillin, clarithromycin, or other macrolides or penicillins).
* History of penicillin allergy.
* Concomitant use with cisapride, pimozide, or ergotamine/dihydroergotamine due to risk of serious cardiac events.
* Severe hepatic impairment (for clarithromycin).
## Adverse Effects
Common adverse effects include diarrhea, taste disturbances, nausea, headache, rash, and abdominal pain. Serious adverse effects include Clostridioides difficile-associated diarrhea, hypersensitivity reactions (including anaphylaxis), Stevens-Johnson syndrome, toxic epidermal necrolysis, QT prolongation (especially with clarithromycin), and severe hepatic reactions.
## Key Drug Interactions
* **Lansoprazole:** May decrease the absorption of drugs that require acidic gastric pH (e.g., ketoconazole, itraconazole, digoxin). May affect metabolism of drugs via CYP2C19 and CYP3A4.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. May interact with allopurinol (increased rash risk).
* **Clarithromycin:** Potent inhibitor of CYP3A4. Significant interactions possible with statins (rhabdomyolysis risk), colchicine, warfarin, carbamazepine, theophylline, triazolam, and certain antiretrovirals. Avoid concomitant use with lovastatin and simvastatin.
## Monitoring
Monitor for signs and symptoms of allergic reactions, severe diarrhea, and C. difficile infection. Assess for treatment efficacy via urea breath test or stool antigen testing after completion of therapy. Monitor electrolytes and ECG if risk factors for QT prolongation are present.
## Clinical Pearls
* This combination is typically prescribed for 10-14 days. Adherence to the full course is crucial for eradication.
* Administer the contents of the packet with water; do not chew or crush the granules.
* Inform patients about potential side effects, especially diarrhea and taste changes, and when to seek medical attention.
* Consider antibiotic resistance patterns in your region when prescribing *H. pylori* eradication regimens.
