# Sompraz hp
## Overview
Sompraz hp is a combination product containing pantoprazole, amoxicillin, and clarithromycin. It is a proton pump inhibitor (PPI), a beta-lactam antibiotic, and a macrolide antibiotic, respectively.
## Primary Indications
H. pylori eradication to reduce the risk of duodenal ulcer recurrence.
## Adult Dosing
The typical adult dose is one bottle (containing pantoprazole 40 mg, amoxicillin 1000 mg, and clarithromycin 500 mg) twice daily for 7 days. Specific dosing regimens may vary based on local guidelines and patient factors.
## Pediatric Dosing
Pediatric dosing is not established for Sompraz hp.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment for pantoprazole is recommended in mild to moderate renal impairment. In severe renal impairment, caution is advised. Amoxicillin and clarithromycin may require dose adjustments based on renal function.
* **Hepatic Impairment:** For pantoprazole, a maximum daily dose of 20 mg is recommended in moderate to severe hepatic impairment. Clarithromycin may require dose reduction in severe hepatic impairment.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, other macrolides, or penicillins.
* History of penicillin-associated jaundice or hepatic dysfunction.
* Concurrent use with rifabutin, rifampicin, or cisapride.
* Congenital QT prolongation.
## Adverse Effects
Common adverse effects include diarrhea, nausea, abdominal pain, headache, dysgeusia (altered taste), and rash. Serious adverse effects can include Clostridioides difficile-associated diarrhea, hypersensitivity reactions, Stevens-Johnson syndrome/toxic epidermal necrolysis, QT prolongation, and hepatic dysfunction.
## Key Drug Interactions
* **Pantoprazole:** Can alter the absorption of drugs that rely on gastric pH (e.g., ketoconazole, itraconazole, digoxin, certain antiretrovirals). Warfarin (increased INR).
* **Amoxicillin:** May decrease the effectiveness of oral contraceptives. Probenecid can increase amoxicillin levels.
* **Clarithromycin:** Potent CYP3A4 inhibitor, increasing levels of drugs metabolized by this enzyme (e.g., statins, benzodiazepines, calcium channel blockers, colchicine, warfarin, theophylline). Avoid concurrent use with cisapride, pimozide, and ergotamine/dihydroergotamine. May increase INR with warfarin.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Assess for efficacy (eradication of H. pylori) with appropriate testing post-treatment.
* Monitor for Clostridioides difficile-associated diarrhea.
* Monitor liver function tests, especially with prolonged use or in patients with pre-existing liver disease.
* Monitor electrolytes, particularly potassium and magnesium, in patients at risk for QT prolongation.
## Clinical Pearls
* Administer as a complete course of therapy.
* Shake the bottle well before each dose.
* May be taken with or without food.
* Ensure patient adherence to the full 7-day regimen.
