Sompraz%25252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252520hp is a brand name for pantoprazole, a proton pump inhibitor (PPI).
### Primary Indications
* Gastroesophageal reflux disease (GERD)
* Healing of erosive esophagitis
* Pathological hypersecretory conditions (e.g., Zollinger-Ellison syndrome)
### Adult Dosing
* **GERD and Healing of Erosive Esophagitis:** 40 mg orally once daily.
* **Pathological Hypersecretory Conditions:** 40 mg orally twice daily, with dose titration based on clinical response. Maximum dose is typically 80 mg daily, divided into two doses.
### Pediatric Dosing
* **GERD and Erosive Esophagitis (ages 5 to 16 years):** 20 mg to 40 mg orally once daily. The specific dose depends on the severity of the condition and body weight. Dosing for children under 5 years is not established.
### Dose Adjustments
No dose adjustment is typically required for hepatic impairment, though caution is advised with severe impairment. No dose adjustment is typically required for renal impairment.
### Contraindications
Hypersensitivity to pantoprazole or any component of the formulation.
### Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, and dizziness. Long-term use may be associated with an increased risk of *Clostridium difficile*-associated diarrhea, bone fractures, and vitamin B12 deficiency.
### Key Drug Interactions
* **Drugs requiring gastric acid for absorption:** PPIs can decrease absorption of drugs like ketoconazole, itraconazole, iron salts, and certain HIV protease inhibitors (e.g., atazanavir, nelfinavir).
* **Methotrexate:** PPIs may increase methotrexate levels.
* **CYP2C19 Substrates:** Pantoprazole is a moderate inhibitor of CYP2C19, which may increase levels of drugs metabolized by this enzyme (e.g., clopidogrel, though clinical significance is debated).
### Monitoring
* Monitor for relief of GERD symptoms.
* Consider monitoring for *C. difficile* infection, especially in patients with diarrhea.
* Long-term therapy may warrant monitoring for vitamin B12 levels and bone mineral density.
### Clinical Pearls
* Administer 30-60 minutes before a meal.
* Pantoprazole can be taken with or without food.
* For patients with difficulty swallowing, pantoprazole delayed-release tablets can be mixed with 100 mL of applesauce, vegetable juice, or mashed potatoes. Do not crush or chew the intact tablet.
