# Sompraz hp
## Overview
Sompraz hp is a combination medication containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori* infection. Pantoprazole is a proton pump inhibitor (PPI), amoxicillin is a penicillin antibiotic, and clarithromycin is a macrolide antibiotic.
## Primary Indications
* Eradication of *Helicobacter pylori* infection in patients with peptic ulcer disease or a history of peptic ulcer disease.
## Adult Dosing
The typical adult dose is one kit taken twice daily for 7 to 14 days. Each kit contains:
* Pantoprazole 40 mg delayed-release tablet
* Amoxicillin 500 mg capsule
* Clarithromycin 500 mg tablet or capsule
**Specific regimen:**
* Take pantoprazole 40 mg twice daily before breakfast.
* Take amoxicillin 500 mg twice daily with meals.
* Take clarithromycin 500 mg twice daily with meals.
The duration of therapy is typically 7 to 14 days, as determined by the prescribing clinician.
## Pediatric Dosing
Pediatric dosing for Sompraz hp is not established. Dosing for individual components in pediatric patients should follow specific pediatric guidelines and expert consultation.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment is recommended for pantoprazole or amoxicillin in mild to moderate renal impairment. However, caution is advised, and severe renal impairment may require dose adjustments for amoxicillin. Clarithromycin requires dose adjustment in moderate to severe renal impairment (see Clarithromycin prescribing information).
* **Hepatic Impairment:** No specific dose adjustment is recommended for pantoprazole in mild to moderate hepatic impairment. Use with caution in severe hepatic impairment. Clarithromycin requires dose adjustment in moderate to severe hepatic impairment (see Clarithromycin prescribing information).
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, other benzimidazoles, penicillins, macrolides, or any component of the formulation.
* History of severe hypersensitivity reactions to beta-lactam antibiotics.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concurrent use with ergotamine derivatives.
* Patients with a history of QT prolongation or ventricular arrhythmias.
* Severe renal impairment when used with omeprazole and clarithromycin (though this specific combination is not Sompraz hp, highlights potential drug interactions).
## Adverse Effects
Common adverse effects may include:
* **Gastrointestinal:** Diarrhea, nausea, abdominal pain, constipation, dry mouth, dyspepsia.
* **Sensory:** Taste disturbances (metallic taste).
* **Dermatologic:** Rash, pruritus.
* **Other:** Headache, dizziness, fungal infections.
* **Serious:** Anaphylaxis, severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis), *Clostridioides difficile*-associated diarrhea, QT prolongation, liver dysfunction, hearing impairment (reversible), tendon rupture (with fluoroquinolones, not directly with this combo but a class warning for macrolides).
## Key Drug Interactions
* **Pantoprazole:** Can affect the absorption of drugs dependent on gastric pH (e.g., ketoconazole, itraconazole, digoxin). May interact with methotrexate.
* **Amoxicillin:** May decrease the effectiveness of oral contraceptives. Can cause a disulfiram-like reaction with alcohol. May interact with allopurinol, increasing the risk of rash.
* **Clarithromycin:** Significant interactions due to CYP3A4 inhibition. Avoid concurrent use with colchicine, ergotamine, dihydroergotamine, lovastatin, simvastatin, rifampin. May increase serum concentrations of numerous drugs including warfarin, theophylline, carbamazepine, cyclosporine, tacrolimus, sirolimus, and certain statins. Potential for QT prolongation when used with other QT-prolonging agents.
## Monitoring
* **Effectiveness:** Monitor for symptom resolution of peptic ulcer disease or duodenal ulcer. Post-treatment *H. pylori* testing (e.g., urea breath test, stool antigen test) is recommended 4-6 weeks after completion of therapy to confirm eradication.
* **Safety:** Monitor for signs and symptoms of hypersensitivity reactions, severe skin reactions, *C. difficile*-associated diarrhea, and liver dysfunction. Monitor electrolytes if GI losses are significant. Assess for signs of QT prolongation.
## Clinical Pearls
* Administer pantoprazole 40 mg once daily before breakfast.
* Administer amoxicillin 500 mg and clarithromycin 500 mg twice daily with meals.
* Complete the full course of therapy, even if symptoms improve.
* Emphasize the importance of compliance with the prescribed regimen for successful eradication.
* Advise patients to report any new or worsening symptoms, or signs of adverse effects immediately.
* Ensure patients are aware of potential drug interactions and consult their pharmacist or physician before starting any new medications.
