# Sompraz hp
## Overview
Sompraz hp is a combination product containing esomeprazole, amoxicillin, and clarithromycin. Esomeprazole is a proton pump inhibitor (PPI), while amoxicillin and clarithromycin are antibiotics.
## Primary Indications
* Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
* **Sompraz hp:** One blister pack (containing esomeprazole 20 mg, amoxicillin 1 g, and clarithromycin 500 mg) taken orally twice daily for 7 days.
## Pediatric Dosing

* **Esomeprazole:** Can decrease the absorption of drugs requiring gastric acidity for absorption (e.g., ketoconazole, itraconazole, iron salts). May increase levels of drugs metabolized by CYP2C19 (e.g., clopidogrel, citalopram).
* **Clarithromycin:** Potent inhibitor of CYP3A4. Numerous interactions are possible, including with statins (lovastatin, simvastatin), ergot alkaloids, cisapride, and certain benzodiazepines. May affect theophylline and carbamazepine levels.
* **Amoxicillin:** May decrease the effectiveness of oral contraceptives. Probenecid can increase and prolong blood levels of amoxicillin.
## Monitoring
* Assess for signs and symptoms of *H. pylori* eradication (e.g., resolution of ulcer symptoms) as appropriate.
* Monitor for signs of hypersensitivity reactions or severe adverse effects, including *C. difficile* infection.
* Monitor hepatic function, especially with prolonged clarithromycin use or in patients with hepatic impairment.
## Clinical Pearls
* This combination therapy is designed to eradicate *H. pylori*. Adherence to the full 7-day course is crucial for treatment success.
* Patients should be advised to take the medication as prescribed and report any new or worsening symptoms immediately.
* The amoxicillin component may be a concern for patients with penicillin allergy.
