# Sompraz HP
## Overview
Sompraz HP is a combination product containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori*.
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
**Standard Regimen:**
* Pantoprazole 40 mg orally twice daily for 10-14 days.
* Amoxicillin 1000 mg orally twice daily for 10-14 days.
* Clarithromycin 500 mg orally twice daily for 10-14 days.
## Pediatric Dosing
* Specific pediatric dosing for Sompraz HP is not established. Dosing should be guided by current pediatric guidelines and expert consultation.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is generally recommended for pantoprazole in mild to moderate renal impairment. Clarithromycin and amoxicillin may require dose adjustment in severe renal impairment; consult specific drug monographs.
* **Hepatic Impairment:** Pantoprazole dose may need to be reduced in severe hepatic impairment; typically, 20 mg once daily. Consult specific drug monographs for clarithromycin and amoxicillin.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or other macrolides.
* History of penicillin allergy.
* Concurrent use with rifampin, cisapride, pimozide, or ergotamine derivatives (due to clarithromycin).
## Adverse Effects
Common adverse effects include diarrhea, nausea, abdominal pain, headache, dizziness, and taste disturbances. Serious adverse effects can include *Clostridioides difficile*-associated diarrhea, allergic reactions, QT prolongation (clarithromycin), and tendon rupture (fluoroquinolones, not in this product, but important for PPIs).
## Key Drug Interactions
* **Pantoprazole:** Clopidogrel (reduced efficacy), drugs affected by gastric pH.
* **Amoxicillin:** Methotrexate (increased toxicity), oral contraceptives (reduced efficacy), warfarin (potential increased INR).
* **Clarithromycin:** CYP3A4 substrates (e.g., statins, warfarin, calcium channel blockers, cyclosporine, tacrolimus, theophylline, digoxin) - increased levels and potential toxicity.
## Monitoring
* Monitor for signs and symptoms of *H. pylori* eradication (e.g., resolution of ulcer symptoms).
* Monitor for adverse effects, particularly diarrhea and signs of hypersensitivity.
* Assess renal and hepatic function if clinically indicated.
## Clinical Pearls
* Administer all components of Sompraz HP for the full 10-14 day course as prescribed.
* Pantoprazole should be taken 30 minutes before a meal.
* Patients with a history of penicillin allergy should be managed with extreme caution or an alternative regimen.
* Urea breath test or stool antigen test is recommended to confirm eradication 4-6 weeks after completing therapy.
