# Sompraz hp
## Overview
Sompraz hp is a combination product containing omeprazole (a proton pump inhibitor) and antibiotics (amoxicillin and clarithromycin) used for the eradication of *Helicobacter pylori* infection.
## Primary Indications
Eradication of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
* **Standard Regimen:**
* Omeprazole 20 mg orally twice daily
* Amoxicillin 1 gram orally twice daily
* Clarithromycin 500 mg orally twice daily
* **Duration:** Typically for 10 to 14 days.
* **Maximum Dose:** Not specified for the combination product; however, standard monotherapy doses for individual components apply.
## Pediatric Dosing
* Sompraz hp is generally **not recommended** for use in pediatric patients due to limited data and potential for adverse effects. Specific pediatric dosing for the combination is not established.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustments are typically recommended for omeprazole, amoxicillin, or clarithromycin within this combination for mild to moderate renal impairment. Severe renal impairment may require dose adjustments for amoxicillin and clarithromycin, consult specific drug monographs.
* **Hepatic Impairment:** Omeprazole dose reduction may be considered in severe hepatic impairment. Clarithromycin may require dose adjustment in moderate to severe hepatic impairment.
## Contraindications
* Hypersensitivity to omeprazole, amoxicillin, clarithromycin, or other macrolides.
* History of allergic reactions to penicillins.
* Concurrent use with ergotamine or dihydroergotamine (due to clarithromycin).
* Severe renal or hepatic impairment (relative contraindication or caution for individual components).
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, abdominal pain, taste disturbances, headache, rash.
* **Serious:** *Clostridium difficile*-associated diarrhea, severe allergic reactions (anaphylaxis), QT prolongation (clarithromycin), hepatotoxicity, potential for drug resistance development.
## Key Drug Interactions
* **Omeprazole:** May decrease the absorption of drugs requiring acidic environments (e.g., ketoconazole, iron salts). May increase levels of certain drugs metabolized by CYP2C19 (e.g., clopidogrel, phenytoin).
* **Clarithromycin:** Potent inhibitor of CYP3A4. Can significantly increase serum concentrations of drugs metabolized by CYP3A4 (e.g., statins, warfarin, benzodiazepines, colchicine, carbamazepine, theophylline). Avoid concomitant use with certain drugs (e.g., ticagrelor, ranolazine).
* **Amoxicillin:** May decrease the efficacy of oral contraceptives.
## Monitoring
* Assess for signs and symptoms of *H. pylori* eradication (e.g., relief of ulcer symptoms).
* Monitor for adverse effects, particularly diarrhea, rash, and signs of hypersensitivity.
* Consider monitoring liver function tests periodically, especially with prolonged use or in patients with pre-existing hepatic conditions.
* Assess adherence to the regimen.
## Clinical Pearls
* Administer the combination product as prescribed for the full duration to ensure eradication and minimize resistance.
* Educate patients on potential side effects and the importance of completing the full course of antibiotics.
* If amoxicillin or clarithromycin allergies are present, alternative eradication regimens should be considered.
* Bismuth subsalicylate-containing quadruple therapy is an alternative regimen for *H. pylori* eradication, especially in areas with high clarithromycin resistance.
