# Sompraz HP
## Overview
Sompraz HP is a combination medication containing pantoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori*.
## Primary Indications
Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease.
## Adult Dosing
The standard adult dose is one Sempraz HP capsule taken orally twice daily for 7 days. Each capsule contains pantoprazole 40 mg, amoxicillin 500 mg, and clarithromycin 500 mg.
## Pediatric Dosing
Dosing for pediatric patients is not established and should be determined by a specialist based on weight and clinical indication.
## Dose Adjustments
No dose adjustment is generally required for renal or hepatic impairment for the combination product. However, individual components may require adjustment if used separately.
## Contraindications
Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or any component of the formulation. History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use. Concomitant use with cisapride, ergotamine derivatives, midazolam, or pimozide due to risk of QT prolongation. Severe renal impairment (for amoxicillin component).
## Adverse Effects
Common adverse effects include diarrhea, nausea, abdominal pain, headache, dysgeusia (altered taste), and rash. Clarithromycin can cause QT prolongation and *Clostridioides difficile*-associated diarrhea. Amoxicillin can cause hypersensitivity reactions.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Clarithromycin is a potent CYP3A4 inhibitor. May increase serum concentrations of drugs metabolized by CYP3A4 (e.g., warfarin, certain statins, benzodiazepines, calcium channel blockers).
* **Penicillin Allergy:** Cross-reactivity with other beta-lactam antibiotics.
* **Proton Pump Inhibitors:** May affect absorption of drugs dependent on gastric pH.
## Monitoring
Monitor for signs of hypersensitivity reactions, gastrointestinal side effects (especially *C. difficile* diarrhea), and liver function abnormalities. Assess for *H. pylori* eradication post-treatment.
## Clinical Pearls
* Administer as a full 7-day course, even if symptoms improve.
* Swallow capsules whole; do not crush or chew.
* Ensure adequate hydration.
* Advise patients to report severe diarrhea or signs of allergic reaction immediately.
