# Sompraz HP
## Overview
Sompraz HP is a combination product containing lansoprazole, amoxicillin, and clarithromycin, used for the eradication of *Helicobacter pylori*.
## Primary Indications
* Treatment of *Helicobacter pylori* infection in patients with duodenal or gastric ulcers.
## Adult Dosing
* **Sompraz HP:** One capsule (containing lansoprazole 30 mg, amoxicillin 500 mg, and clarithromycin 250 mg) taken orally twice daily for 7 to 14 days.
* **Alternative (if Sompraz HP is not available):**
* Lansoprazole 30 mg orally twice daily
* Amoxicillin 500 mg orally twice daily
* Clarithromycin 250 mg orally twice daily
* Dosing duration is 7 to 14 days.
## Pediatric Dosing
* **Lansoprazole:**
* **1-17 years:** 15 mg once daily for up to 8 weeks for erosive esophagitis. Dosing for *H. pylori* eradication is not well-established for this age group and typically relies on specialized pediatric gastroenterology guidance.
* **Amoxicillin:**
* **Infants and Children < 40 kg:** 20-40 mg/kg/day divided into 2-3 doses. For severe infections, up to 80-90 mg/kg/day.

* **Clarithromycin:**
* **Children:** 7.5 mg/kg orally twice daily, not to exceed 500 mg twice daily.

## Dose Adjustments

## Key Drug Interactions
* **Lansoprazole:** May decrease the absorption of drugs requiring acidic gastric pH (e.g., ketoconazole, itraconazole, iron salts). May affect metabolism of clopidogrel.
* **Clarithromycin:** Potent inhibitor of CYP3A4. Increases serum concentrations of drugs metabolized by CYP3A4, including statins, warfarin, carbamazepine, phenytoin, cyclosporine, tacrolimus, midazolam, and certain calcium channel blockers. Avoid concomitant use with colchicine, pimozide, cisapride, and ergotamine/dihydroergotamine.
* **Amoxicillin:** May decrease the efficacy of oral contraceptives. Probenecid can increase and prolong serum levels of amoxicillin.
## Monitoring
* Symptom assessment for ulcer healing and *H. pylori* eradication.
* Monitor for signs of hypersensitivity reactions.
* Monitor for diarrhea, especially signs of *C. difficile*.
* For prolonged therapy or in patients with hepatic impairment, liver function tests may be considered.

* Administer Sompraz HP as a complete regimen; do not substitute individual components without consulting a healthcare provider.
