# Sompraz hp
## Overview
Sompraz hp is a fixed-dose combination product containing pantoprazole (a proton pump inhibitor or PPI) and amoxicillin and clarithromycin (antibiotics). It is designed for the eradication of *Helicobacter pylori* (H. pylori).
## Primary Indications
* Treatment of *Helicobacter pylori* infection in patients with peptic ulcer disease or at high risk for peptic ulcer disease.
## Adult Dosing
**Standard Regimen:**
* One packet twice daily for 10 to 14 days.
* Each packet typically contains:
* Pantoprazole 40 mg
* Amoxicillin 1000 mg
* Clarithromycin 500 mg
Dosing is usually given in the morning and evening.
## Pediatric Dosing
Pediatric dosing for Sompraz hp is not established. Dosing of individual components (pantoprazole, amoxicillin, clarithromycin) in pediatric patients should be determined based on weight and clinical guidelines for each agent separately.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment for pantoprazole is typically required unless severe impairment. Amoxicillin and clarithromycin may require dose adjustments in severe renal impairment; consult specific prescribing information for these agents.
* **Hepatic Impairment:** Pantoprazole dose reduction may be considered in patients with severe hepatic impairment. Clarithromycin may require dose reduction in moderate to severe hepatic impairment. Consult specific prescribing information.
## Contraindications
* Hypersensitivity to pantoprazole, amoxicillin, clarithromycin, or any component of the formulation.
* History of severe hypersensitivity reactions to penicillins, cephalosporins, or other beta-lactam antibiotics.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concurrent use with rifabutin, rifampin, or other strong CYP3A4 inducers (for clarithromycin).
* Concurrent use with colchicine (for clarithromycin).
* Use in patients with a creatinine clearance less than 30 mL/min (for clarithromycin).
## Adverse Effects
Common adverse effects may include diarrhea, nausea, abdominal pain, headache, rash, and taste perversion. Serious adverse effects can include *Clostridioides difficile*-associated diarrhea, severe allergic reactions, liver dysfunction, and QT prolongation (clarithromycin).
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Clarithromycin is a moderate CYP3A4 inhibitor and can increase the concentration of drugs metabolized by CYP3A4 (e.g., statins, warfarin, certain immunosuppressants). Concurrent use with strong CYP3A4 inducers (e.g., rifampin) is contraindicated.
* **Pantoprazole:** Can decrease the absorption of drugs that require acidic gastric pH (e.g., ketoconazole, itraconazole, certain HIV protease inhibitors).
* **Amoxicillin:** May interfere with urine glucose testing. Probenecid can decrease renal excretion of amoxicillin.
* **Warfarin:** Monitor INR closely as amoxicillin and clarithromycin can potentially affect warfarin levels.
* **Digoxin:** Clarithromycin can increase digoxin levels.
## Monitoring
* Assess for signs and symptoms of *H. pylori* eradication (e.g., resolution of ulcer symptoms).
* Monitor for signs and symptoms of allergic reactions.
* Monitor for diarrhea, especially *C. difficile*-associated diarrhea.
* Monitor liver function tests periodically, especially in patients with pre-existing liver disease or those on prolonged therapy.
* If using concurrently with warfarin, monitor INR.
## Clinical Pearls
* This is a combination product, and the duration of therapy is fixed (typically 10-14 days). Ensure adequate patient adherence.
* Patient counseling should include instructions on how to take the medication (e.g., timing of doses, taking with food), potential side effects, and when to seek medical attention.
* Consider local antibiotic resistance patterns for *H. pylori* when prescribing this regimen.
* Ensure patients are not allergic to penicillin or macrolide antibiotics.
