# Sompraz HP
## Overview
Sompraz HP is a combination medication containing omeprazole (a proton pump inhibitor) and amoxicillin and clarithromycin (antibiotics). It is used for the eradication of *Helicobacter pylori* (*H. pylori*) infection.
## Primary Indications
Eradication of *Helicobacter pylori* in patients with peptic ulcer disease.
## Adult Dosing
The standard adult dose is one kit taken twice daily for 10 days. Each kit typically contains:
* Omeprazole 20 mg
* Amoxicillin 500 mg
* Clarithromycin 250 mg

## Pediatric Dosing
Pediatric dosing is not well-established for Sompraz HP as a fixed combination. Dosing of individual components would be based on age, weight, and *H. pylori* eradication guidelines.

No dose adjustments are typically required for renal or hepatic impairment for the standard course of treatment, but caution is advised, especially with clarithromycin in severe hepatic impairment. Refer to individual drug monographs for specific guidance.
## Contraindications
* Hypersensitivity to omeprazole, amoxicillin, clarithromycin, or other macrolides or penicillins.
* History of cholestatic jaundice/hepatic dysfunction associated with prior clarithromycin use.
* Concomitant use with rifabutin, cisapride, pimozide, or statins metabolized by CYP3A4 (e.g., simvastatin, lovastatin).
* Severe renal impairment (especially for amoxicillin).
## Adverse Effects
Common adverse effects include diarrhea, nausea, abdominal pain, taste disturbances, headache, and rash. Clarithromycin can cause QT prolongation. Amoxicillin can cause hypersensitivity reactions, including anaphylaxis.
## Key Drug Interactions
* **Clarithromycin:** Potent CYP3A4 inhibitor, leading to increased levels of colchicine, statins, warfarin, benzodiazepines, and others. May interact with antacids.
* **Omeprazole:** Inhibits CYP2C19 and CYP3A4, potentially increasing levels of clopidogrel, phenytoin, methotrexate, and others.
* **Amoxicillin:** May decrease the effectiveness of oral contraceptives.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for diarrhea, which may indicate *Clostridioides difficile* infection.
* Assess adherence to the full 10-day course.
* Test for *H. pylori* eradication (e.g., urea breath test, stool antigen test) at least 4 weeks after completing therapy.
## Clinical Pearls
* Ensure patients understand the importance of taking all medications in the kit as directed for the full 10 days.
* Advise patients to take omeprazole capsules 30 minutes before a meal.
* Amoxicillin and clarithromycin should be taken with food or milk to minimize gastrointestinal upset.
