# Sompraz HP
## Overview
Sompraz HP is a prescription "Helicobacter Pylori kit" (a combination pack). It typically contains a fixed-dose combination of **Esomeprazole** (a proton pump inhibitor), **Amoxicillin** (a penicillin antibiotic), and **Clarithromycin** (a macrolide antibiotic).
## Primary Indications
Treatment of *Helicobacter pylori* infection to reduce the risk of duodenal ulcer recurrence.
## Adult Dosing
Standard therapy consists of the following, usually taken twice daily (morning and evening) for 7 to 14 days, depending on local prescribing guidelines:
* **Esomeprazole:** 40 mg twice daily.
* **Amoxicillin:** 1000 mg twice daily.
* **Clarithromycin:** 500 mg twice daily.
* *Note:* Always follow the specific blister pack instructions provided in the kit.
## Pediatric Dosing
* **Not indicated.** Safety and efficacy have not been established in the pediatric population.
## Dose Adjustments
* **Renal Impairment:** Amoxicillin dose usually requires reduction if CrCl <30 mL/min. Clarithromycin may require adjustment in severe renal impairment (CrCl <30 mL/min).
* **Hepatic Impairment:** Caution with Clarithromycin; may require dose reduction or avoidance in severe liver disease. Esomeprazole does not typically require adjustment in mild-to-moderate impairment, but use caution in severe hepatic disease (max 20 mg/day).
## Contraindications
* Hypersensitivity to any component (Esomeprazole, Amoxicillin, Clarithromycin).
* Patients with a history of cholestatic jaundice or hepatic dysfunction associated with clarithromycin use.
* **Concurrent use** with drugs known to cause QT prolongation (e.g., pimozide, astemizole, terfenadine, cisapride) due to clarithromycin.
* History of penicillin allergy (severe).
## Adverse Effects
* **Common:** Diarrhea, dysgeusia (metallic taste), nausea, abdominal pain, headache, and dizziness.
* **Serious:** *Clostridioides difficile*-associated diarrhea (CDAD), hypersensitivity reactions (anaphylaxis/Stevens-Johnson syndrome), QTc prolongation, and hepatotoxicity.
## Key Drug Interactions
* **CYP3A4 Substrates:** Clarithromycin is a potent CYP3A4 inhibitor; increased levels of drugs like statins (e.g., simvastatin/lovastatin – increased risk of rhabdomyolysis), benzodiazepines, and warfarin may occur.
* **pH-Dependent Drugs:** Esomeprazole increases gastric pH, reducing absorption of atazanavir, nelfinavir, and ketoconazole.
* **Methotrexate:** PPIs may increase methotrexate levels.
## Monitoring
* Monitor for signs of *C. difficile* diarrhea (persistent watery/bloody stools).
* Monitor for signs of hypersensitivity.
* Baseline and periodic LFTS if on long-term therapy or higher risks.
* Assess for resolution of symptoms; confirm *H. pylori* eradication 4 weeks after completion of therapy via urea breath test or stool antigen testing.
## Clinical Pearls
* **Adherence:** High success rates depend on strict patient adherence to the full 7–14 day course despite gastrointestinal side effects.
* **Resistance:** Increasing resistance of *H. pylori* to clarithromycin has reduced efficacy in certain geographic regions. Consult local antibiograms.
* **Administration:** Esomeprazole dosing is usually optimal at least 30–60 minutes before meals.
