# Oxymetazoline Nasal Spray
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical decongestant. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Sinex Nasal Spray (0.05%):** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Sinex Ultra/Severe Congestion (0.05%):** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Sinex Long-Acting (0.05%):** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
## Pediatric Dosing
* **Children 6 years and older:** 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Children under 6 years:** Consult a healthcare professional. Safety and efficacy are not established.
## Dose Adjustments
* No dose adjustments are typically necessary based on renal or hepatic impairment. However, caution is advised in patients with severe underlying conditions.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use within 14 days of discontinuing a monoamine oxidase inhibitor (MAOI) due to risk of hypertensive crisis.
* Patients with narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common:** Rebound congestion (rhinitis medicamentosa) with prolonged or excessive use, headache, dizziness, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs) and within 14 days of MAOI discontinuation:** Risk of hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate cardiovascular effects.
* **Other sympathomimetic agents:** May cause additive pressor effects.
## Monitoring
* Monitor for signs of systemic absorption, especially in patients with cardiovascular disease or hypertension.
* Assess for rebound congestion with prolonged use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
* Advise patients to blow their nose gently after administration.
* Store at room temperature, away from heat and direct light.
