# Oxymetazoline Nasal Spray
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical vasoconstrictor.
## Primary Indications
* Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Sinex (0.05% nasal spray):** 2 to 3 sprays in each nostril every 10 to 12 hours as needed.
* **Do not exceed 2 doses in any 24-hour period.**
## Pediatric Dosing
* **Children 6 to 12 years:** 2 to 3 sprays in each nostril every 10 to 12 hours as needed.
* **Children under 6 years:** Consult a healthcare provider.
## Dose Adjustments
* No specific dose adjustments are necessary for renal or hepatic impairment. However, caution is advised in patients with hypertension, heart disease, or hyperthyroidism.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use within 14 days of stopping a monoamine oxidase inhibitor (MAOI).
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Less Common:** Increased blood pressure, headache, nervousness, insomnia, palpitations, rebound nasal congestion (with prolonged use).
* **Rare:** Allergic reactions.
## Key Drug Interactions
* **MAOIs:** Concomitant use can cause severe hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
## Monitoring
* Monitor for rebound congestion, especially with use beyond 3 days.
* Monitor blood pressure in patients with pre-existing cardiovascular conditions.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound nasal congestion (rhinitis medicamentosa).
* Advise patients to avoid spraying in eyes.
* If pregnant or breastfeeding, consult a healthcare provider before use.
