## Sinex (oxymetazoline)
### Overview
Sinex is a nasal spray containing oxymetazoline, a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, leading to decongestion.
### Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
### Adult Dosing
* **Nasal Spray (0.05%):** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
### Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Children under 6 years:** Not recommended. Consult a healthcare professional.
### Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment, but caution is advised in patients with cardiovascular disease.
### Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use within 14 days of discontinuing a monoamine oxidase inhibitor (MAOI) due to risk of hypertensive crisis.
### Adverse Effects
Common: Nasal burning, stinging, irritation, sneezing.
Less Common: Rhinitis medicamentosa (rebound congestion with prolonged use), increased blood pressure, insomnia, nervousness, headache, nausea.
### Key Drug Interactions
* **MAOIs:** Concomitant use or recent discontinuation can lead to hypertensive crisis.
* **Other sympathomimetics (e.g., pseudoephedrine, phenylephrine):** May potentiate cardiovascular effects like increased blood pressure and heart rate.
### Monitoring
* Monitor for rebound congestion (rhinitis medicamentosa) with prolonged or frequent use.
* Monitor blood pressure, especially in patients with pre-existing cardiovascular conditions or those taking MAOIs.
### Clinical Pearls
* Limit use to a maximum of 3 consecutive days to avoid rebound congestion.
